牙周联合正畸对侵袭性牙周炎患者炎症因子及牙周功能的影响

Effects of periodontal combined orthodontic treatment on inflammatory factors and periodontal function in patients with aggressive periodontitis

目的探讨牙周联合正畸对侵袭性牙周炎患者炎症因子及牙周功能的影响。方法选取2019年5月至2020年6月本院收治的82例侵袭性牙周炎患者作为研究对象,采用随机数字表法分为观察组与对照组,各41例。对照组行基础性牙周治疗,观察组在对照组基础上联合正畸治疗,比较两组牙周功能指标、炎症因子水平。结果治疗前,两组牙龈指数(GI)、出血指数(BI)评分和牙周探针深度(PD)比较差异无统计学意义;治疗后,两组GI、BI评分均低于治疗前,PD深于治疗前,且观察组GI、BI评分均低于对照组,PD深于对照组,差异有统计学意义(P<0.05)。治疗前,两组龈沟液肿瘤坏死因子-α(TNF-α)、白细胞介素-6(IL-6)水平比较差异无统计学意义;治疗后,两组TNF-α、IL-6水平均低于治疗前,且观察组低于对照组,差异有统计学意义(P<0.05)。结论牙周联合正畸治疗侵袭性牙周炎效果确切,可减轻炎症反应,改善牙周功能,值得临床推广应用。

Objective To investigate the effects of periodontal combined orthodontic treatment on inflammatory factors and periodontal function in patients with aggressive periodontitis.Methods 82 patients with aggressive periodontitis admitted to our hospital from May 2019 to June 2020 were selected as the study subjects,and they were divided into the observation group and the control group according to the random number table method,with 41 cases in each group.The control group received basic periodontal treatment,and the observation group was combined with orthodontic treatment on the basis of the control group,the periodontal function indexes and inflammatory factors were compared between the two groups.Results Before treatment,there was no statistically significant difference in gingival index(GI),bleeding index(BI)scores,and periodontal probe depth(PD)between the two groups;after treatment,the scores of GI,BI of the two groups were lower than those before treatment,and PD was deeper than that before treatment,and GI and BI scores in the observation group were lower than those in the control group,PD was deeper than that in the control group,the differences were statistically significant(P<0.05).Before treatment,there were no significant differences in the levels of gingival crevicular fluid tumor necrosis factor-α(TNF-α),interleukin-6(IL-6)between the two groups;after treatment,the levels of TNF-αand IL-6 of the two groups were lower than those before treatment,and the observation group was lower than the control group,and the differences were statistically significant(P<0.05).Conclusion Periodontal combined orthodontics treatment of aggressive periodontitis has definite effect,and can reduce inflammatory reaction and improve periodontal function,which is worthy of clinical promotion and application.
