基于快速康复外科护理干预对妇科腹腔镜手术患者术后康复及并发症的影响

Effect of nursing intervention based on fast track surgery on postoperative rehabilitation and complications of patients undergoing gynecological laparoscopic surgery

目的探讨基于快速康复外科护理干预对妇科腹腔镜手术患者术后康复效果及并发症的影响。方法选取2018年6月至2020年6月本院收治的100例妇科腹腔镜手术患者作为研究对象,按照奇偶数法分为对照组与研究组,每组50例。对照组采用常规护理干预,研究组采用快速康复外科护理干预。比较两组术后疼痛数字评分法(NRS)评分、手术前后自我效能量表(GSES)评分、术后恢复情况(肠鸣音恢复时间、肛门排气时间、排便时间、下床活动时间)、住院时间及住院费用、并发症发生情况。结果术后6、24 h,研究组NRS评分均低于对照组,差异有统计学意义(P<0.05);术后3 d,研究组GSES评分高于对照组,差异有统计学意义(P<0.05);研究组肠鸣音恢复时间、肛门排气时间、排便时间、下床活动时间均短于对照组,差异有统计学意义(P<0.05);研究组住院时间短于对照组,且住院费用少于对照组,差异有统计学意义(P<0.05);研究组并发症发生率低于对照组,差异有统计学意义(P<0.05)。结论对妇科腹腔镜手术患者实施快速康复外科护理模式,患者术后恢复效果显著,不仅可缓解其疼痛程度,改善其自我效能感,还可降低并发症发生风险,缩短康复进程,降低住院费用,值得临床推广应用。

Objective To investigate the effect of nursing intervention based on fast track surgery on postoperative rehabilitation and complications of patients undergoing gynecological laparoscopic surgery.Methods A total of 100 patients undergoing laparoscopic gynecological surgery admitted to our hospital from June 2018 to June 2020 were selected as the research subjects,and they were divided into the control group and the study group according to odd-even method,with 50 cases in each group.The control group received routine nursing interventions,and the study group received fast track surgery nursing interventions.The postoperative numerical rating scale(NRS)scores,self-effective energy scale(GSES)score before and after surgery,postoperative recovery(intestinal sounds recovery time,anal exhaust time,defecation time,getting out of bed activity time),hospitalization time,hospitalization expenses,and the incidence of complications were compared between the two groups.Results At 6 and 24 h after surgery,the NRS scores in the study group were lower than that in the control group,and the difference was statistically significant(P<0.05).3 d after surgery,the GSES scores in the study group were higher than that in the control group,and the difference was statistically significant(P<0.05);the intestinal sounds recovery time,anal exhaust time,defecation time,and getting out of bed activity time in the study group were shorter than those in the control group,the differences were statistically significant(P<0.05);the hospitalization time in the study group was shorter than that in the control group,and the hospitalization expenses was less than that in the control group,the differences were statistically significant(P<0.05);the incidence of complication rate in the study group was lower than that in the control group,the difference was statistically significant(P<0.05).Conclusion The implementation of fast track surgery nursing model for patients undergoing laparoscopic gynecological surgery in clinical nursing has significant postoperative recovery effect,which can not only relieve their pain symptoms,improve their self-efficacy,but also reduce the risk of complications,shorten the rehabilitation process,and reduce hospitalization expenses,which is worthy of clinical promotion and application.
