1211份门诊中成药处方点评及分析

Review and Analysis of 1211 Outpatient Prescriptions of Chinese Patent Medicines

目的 了解厦门市中医院门诊中成药处方使用情况,并探讨用药的合理性,以促进临床合理用药。方法 利用医院的精配安全用药智能辅助决策系统抽取2020年1月—2020年12月厦门市中医院门诊西药房仅开具中成药的处方(无同时开具中药饮片或西药),按每月各科室相同比例数,随机抽取约100张,共计1211张。对每张处方平均金额、不合理处方占比、开具不合理处方的医生类型占比和各类不合理处方占比进行统计,并对处方进行回顾性点评与分析。结果 每张处方平均金额120.68元,不合理处方163张(占总处方比13.46%),其中不规范处方48例(占29.45%),用药不适宜处方119例(占73.01%)。结论 厦门市中医院医生对中成药的使用中尚未达到国家卫健委所规定的要求,需加强中医临床基础理论培训,开展处方前置审查,培养中西医结合的临床药师,从而达到提高临床用药合理性的目标。

Objective To study the prescription of Chinese medicines in Xiamen Hospital of Traditional Chinese Medicine and to ex-plore the rationality of medicine use to promote the rational clinical use of medicines.Methods From January 2020 to December 2020,1211 Chinese patent medicine prescriptions(about 100 prescriptions per month)were selected by the intelligent auxiliary deci-sion system of fine dispensing safe drug use of the hospital,and the average amount of each prescription,the proportion of unreason-able prescriptions,the proportion of doctors issuing unreasonable prescriptions and the proportion of various unreasonable prescrip-tions were counted,and the prescriptions were reviewed and analyzed.Results The average amount of each prescription was¥120.68,and there were 163 unreasonable prescriptions(13.46%of the total prescriptions),among which 48 were irregular prescriptions(29.45%)and 119 were inappropriate prescriptions(73.01%).Conclusion Doctors of Xiamen Hospital of Traditional Chinese Medi-cine has not yet met the requirements stipulated by the National Health and Family Planning Commission in the process of using Chi-nese patent medicines.It is necessary to carry out basic theoretical training of traditional Chinese medicine,carry out pre-prescription review,and train clinical pharmacists of integrated traditional Chinese and Western medicine,so as to improve the objectives for ratio-nality of clinical medication use.
