探讨头颅MRI辅助测量共同性斜视角度的准确性与可行性

目的探讨头颅MRI辅助测量显性水平性共同性斜视角度的准确性与可行性。方法选取2018年1月至2022年12月于江西医学高等专科学校第一附属医院眼科行共同性斜视矫正手术治疗的64例显性水平性共同性斜视患者作为研究对象,根据不同斜视情况分为外斜视组(n=40)与内斜视组(n=24)。两组均采用MRI辅助测量斜视度和三棱镜测量斜视度,比较两组MRI辅助测量与三棱镜测量的斜视度及术后眼位情况。结果两组斜视度MRI辅助测量与三棱镜测量均呈正相关(r=0.899、0.956,P<0.001);两组MRI辅助测量和三棱镜测量斜视度具有同等准确性(P<0.025)。两组术后不同时间点正眼位数百分比比较差异无统计学意义。两组术前、术后1 d、1个月、3个月三棱镜测量斜视度比较差异无统计学意义。两组三棱镜测量斜视度时间比较差异有统计学意义(P<0.05);两组三棱镜测量斜视度组间、交互比较差异无统计学意义。组内比较:术后1 d、1个月、3个月,两组三棱镜测量斜视度均高于术前,术后1 d、1个月两组三棱镜测量斜视度均低于术后3个月内,差异有统计学意义。组间比较:两组三棱镜测量斜视度差异无统计意义。结论MRI辅助测量显性水平性共同性斜视角度是一种可行的测量方法,其测量结果与采用三棱镜测量的斜视角度的结果在等效性上具有一致性。

Objective To investigate the accuracy and feasibility of magnetic resonance imaging(MRI)assisted measurement of dominant horizontal concomitant strabismus.Methods From January 2018 to December 2022,64 patients with dominant level concomitant strabismus who received corrective surgery in the Ophthalmology Department of the First Affiliated Hospital of Jiangxi Medical College were selected as the research subjects,and they were divided into extropia group(n=40)and esotropia group(n=24)according to different conditions of strabismus.Both groups used MRI-assisted measurement of strabismus and prism measurement of strabismus.The strabismus and postoperative eye position were compared between the two groups.Results MRI and prism measurements of the two groups were positively correlated(r<1,P<0.05).Both MRI and prism measurements had the same accuracy(P<0.025).There was no significant difference in the percentage of positive eye digits between the two groups at different time points.There was no significant difference in the degree of strabismus measured by prism between the two groups before surgery,1 day after surgery,1 month after surgery and 3 months after surgery.There were statistically significant differences in strabismus measured by prism at different time points between the two groups(P<0.05).There was no significant difference in the degree of strabismus measured by prism 1 day after surgery and 1 month after surgery.The comparison of strabismus measured by prism 1 day after surgery,1 month after surgery and 3 months after surgery were lower than that before surgery,and the comparison of strabismus measured by prism 1 day after surgery and 1 month after surgery were lower than that after 3 months,the difference was statistically significant(P<0.05).There was no significant difference in the degree of strabismus measured by prism 1 month after surgery and 3 months after surgery.The comparison of strabismus measured by prism 1 day after surgery,1 month after surgery and 3 months after surgery were lower than that before surgery,and the comparison of strabismus measured by prism 1 day after surgery was lower than that measured by prism 3 months after surgery,the difference was statistically significant(P<0.05).Conclusion MRI assisted measurement of dominant horizontal concomitant strabismus is a feasible method,and the results of the measurement are consistent with the results of the prism measurement in equivalence.
