输液港置管后异物不适感与肩关节活动度的相关性研究

摘要 目的探讨输液港(PORT)置管术后患者的异物不适感与肩关节活动状况的危险因素及其相关性分析。方法回顾性分析2020年6至11月本院收治的203例肺癌患者的临床资料,采用患者临床资料调查表、异物不适感量表评价患者异物不适感程度、肩关节功能评价量表评价肩关节活动状况,分析异物不适感评分、肩关节功能评价量表评分与临床资料的关系,采用多元线性回归分析患者肩关节功能降低和异物不适感评分的危险因素,采用Spearman相关分析肩关节功能下降与异物不适感的相关性。结果患者平均异物不适感评分为(1.50±1.26)分,肩关节功能评价量表评分(95.56±6.36)分;不同性别、年龄、带管时间、置管侧活动患者异物不适感评分、肩关节功能评价量表评比较差异有统计学意义(P<0.05)。Spearman相关分析显示,肩关节功能评分与疼痛、肩关节活动范围、日常生活活动能力与呈正相关(r>0,P<0.05),与异物不适感总评分呈负相关(r<0,P<0.05)。多元线性回归分析显示,年龄、性别、置管侧活动、带管时间是患者肩关节功能降低的独立危险因素(P<0.05)。多元线性回归分析显示,年龄、性别、置管侧活动、带管时间是异物不适感评分的独立危险因素(P<0.05)。结论PORT置管术后患者肩关节功能降低与异物不适感相关,医护人员应重点评估PORT置管术后患者是否存在存在异物不适感及对其肩关节的功能是否产生影响,及时给予指导,减少对肩关节功能的影响,提升患者的生命质量。 Objective To explore the risk factors and correlation analysis of foreign body discomfort and shoulder joint mobility in patients after implantable venous access port catheterization.Methods The clinical data of 203 patients with lung cancer admitted to our hospital from June to November 2020 were retrospectively analyzed.The clinical data questionnaire,foreign body discomfort scale were used to evaluate the degree of foreign body discomfort in patients,and shoulder joint function evaluation scale was used to evaluate shoulder joint activity status.The relationship between foreign body discomfort score,shoulder joint function evaluation scale score and general data was analyzed.Multiple linear regression analysis were used to analyze the risk factors of shoulder joint function decline and foreign body discomfort,and Spearman correlation were used to analyze the correlation between shoulder joint function decline and foreign body discomfort.Results The average score of foreign body discomfort was(1.50±1.26),and the score of shoulder joint function evaluation scale was(95.56±6.36).There were significant differences in the scores of foreign body discomfort and shoulder joint function evaluation scale among patients with different gender,age,catheter time,side movement(P<0.05).Spearman correlation analysis showed that shoulder joint function score was positively correlated with pain,shoulder joint range of motion and activity of daily living(r>0,P<0.05),and negatively correlated with the total score of foreign body discomfort(r<0,P<0.05).Multiple linear regression analysis showed that age,gender,lateral movement and catheter time were independent risk factors for shoulder joint function reduction(P<0.05).Multiple linear regression analysis showed that age,sex,lateral movement and time were independent risk factors for foreign body discomfort score(P<0.05).Conclusions The reduced shoulder joint function of patients after PORT catheterization is related to foreign body discomfort.Medical staff should focus on assessing whether there is foreign body discomfort in patients after PORT catheterization and whether it has an impact on their shoulder joint function,and give timely guidance to reduce the impact on shoulder joint function and improve the quality of life of patients.
