MRI与CT对不同类型腰椎间盘突出症的鉴别价值分析

Analysis of the diagnostic value ofMRI and CT in different types of lumbar disc herniation

目的分析MRI与CT对不同类型腰椎间盘突出症(LDH)的鉴别价值。方法选取2019年1月至2021年2月本院收治的82例LDH患者作为研究对象,所有患者均经MRI、CT检查,比较MRI与CT对不同类型LDH的诊断结果及征象的诊断结果。结果MRI对破裂型LDH、非破裂型LDH及总检出率均高于CT,差异有统计学意义(P<0.05)。MRI诊断破裂型髓核易位、后缘不规则、低信号带、椎间盘变性患者占比显著高于非破裂型患者,差异有统计学意义(P<0.05)。CT诊断破裂型患者巨大突出、髓核易位、边界模糊、巨大锐角患者占比显著高于非破裂型患者,差异有统计学意义(P<0.05)。结论MRI对不同类型LDH患者诊断率高于CT,CT对LDH患者腰椎位置、形态等显示效果较好,而MRI对椎管内结构显示较好,临床可根据治疗需求选取适合的方法对患者进行检查,或将两种方法联合应用,提高其诊断价值。

Objective To analyze the diagnostic value of MRI and CT in different types of lumbar disc herniation.Methods 82 patients with lumbar disc herniation admitted to our hospital from January 2019 to February 2021 were selected as the study subjects,all patients were examined by MRI and CT,and the diagnostic results of different types of lumbar disc herniation and different types of lumbar disc herniation signs were compared between MRI and CT.Results The detection rate of MRI for ruptured type,non-ruptured type and total were higher than that of CT,the difference was statistically significant(P<0.05).The proportion of patients diagnosed by MRI with ruptured nucleus pulposus translocation,irregular posterior margin,low signal band and disc degeneration was significantly higher than that of non-ruptured patients,the difference was statistically significant(P<0.05).The proportion of patients with large protrusion,nucleus pulposus translocation,blurred boundary and large acute angle diagnosed by CT was significantly higher than that of non-ruptured patients,and the difference was statistically significant(P<0.05).Conclusion MRI has a higher diagnostic rate for patients with different lumbar disc herniation than CT,and CT has a better display effect on the location and shape of lumbar spine in patients with lumbar disc herniation,while MRI has a better display of intramural structure,clinically,patients can be examined by appropriate methods according to treatment needs,or the two methods can be combined for the diagnosis,improve its diagnostic value.
