改良腮腺部分切除术与腮腺浅叶切除术治疗腮腺下极良性肿瘤患者的预后分析

Prognosis analysis of modified partial parotid resection and superficial parotid lobectomy in the treatment of inferior parotid benign tumors

目的分析改良腮腺部分切除术与腮腺浅叶切除术治疗腮腺下极良性肿瘤患者的预后效果。方法选取2017年2月至2020年1月本院收治的84例腮腺下极良性肿瘤患者作为研究对象,根据手术方式不同分为常规组与改良组,每组42例。常规组采用腮腺浅叶切除术治疗,改良组采用改良腮腺部分切除术治疗,比较两组术中出血量、切口长度、术后并发症发生情况及肿瘤复发情况。结果改良组术中出血量少于常规组,切口长度短于常规组,差异有统计学意义(P<0.05)。改良组术后并发症发生率为0.00%,明显低于常规组的9.52%,差异有统计学意义(P<0.05);两组肿瘤复发率比较差异无统计学意义。结论改良腮腺部分切除术与腮腺浅叶切除术均可用于治疗腮腺下极良性肿瘤,清除肿瘤组织,且远期疗效相当,但改良腮腺部分切除术术中出血风险较低,手术创伤较小,术后并发症更少。

Objective To analyze the prognostic effects of modified partial parotid resection and superficial parotid lobectomy in the treatment of inferior parotid benign tumors.Methods 84 patients with inferior parotid extremely benign tumors admitted to our hospital from February 2017 to January 2020 were selected as the study subjects,and they were divided into conventional group and improved group according to different surgical methods,with 42 cases in each group.The conventional group was treated with superficial parotid lobectomy,and the improved group was treated with modified partial parotidectomy.The intraoperative blood loss,incision length,postoperative complications and tumor recurrence were compared between the two groups.Results The intraoperative blood loss in the improved group was less than that in the conventional group,and the incision length was shorter than that in the conventional group,and the differences were significant(P<0.05).The incidence of postoperative complications in the improved group was 0.00%,which was significantly lower than 9.52%in the conventional group,and the difference was statistically significant(P<0.05).There was no significant difference in tumor recurrence rate between the two groups.Conclusion Both modified partial parotid resection and superficial parotid lobectomy can treat inferior parotid benign tumors and remove tumor tissue with similar long-term efficacy,but modified partial parotid resection has lower risk of intraoperative bleeding,less surgical trauma and fewer postoperative complications.
