人乳头状瘤病毒核酸定量检测与宫颈细胞学病理特征的相关研究

摘要 目的探讨人乳头状瘤病毒(HPV)核酸定量检测与宫颈细胞学病理特征的相关性。方法选取2018年5月至2021年4月本院收治的82例宫颈病变患者作为研究对象。所有患者均接受HPV核酸定量检测并分析单位细胞内HPV病毒载量,分析不同宫颈病变患者病理特征与HPV阳性的关系,比较不同病理分型患者HPV病毒载量,分析HPV病毒载量与宫颈上皮内瘤变(CIN)分级的相关性。结果HPV核酸定量检测结果显示,82例宫颈病变患者中,HPV阳性人数61例,其中低级别鳞状上皮内病变(LSIL)、高级别鳞状上皮内病变(HSIL)、宫颈癌患者中HPV阳性人数分别为19、18、24例。不同病理分型、宫旁侵润患者HPV阳性占比比较差异有统计学意义(P<0.05);不同年龄、肿瘤最大径、淋巴结转移患者HPV阳性占比比较差异无统计学意义。随着患者宫颈病变程度提高HPV病毒载量呈升高趋势,差异有统计学意义(P<0.05);且HSIL、宫颈癌患者HPV病毒载量均高于LSIL患者,宫颈癌患者高于HSIL患者,差异有统计学意义(P<0.05)。Pearson相关分析结果显示,HPV病毒载量与CIN分级呈正相关(r=0.767,P<0.05)。结论HPV核酸定量检测技术可对宫颈病变患者HPV感染情况及病毒载量进行准确分析,有助于临床对检查者宫颈病变程度及癌变风险做出准确诊断。 Objective To explore the correlation between quantitative detection of human papillomavirus(HPV)nucleic acid quantitative detection and pathological features of cervical cytology.Methods 82 patients with cervical lesions admitted to our hospital from May 2018 to April 2021 were were selected as the research subjects.All patients underwent HPV nucleic acid quantitative detection and analyzed the HPV viral load per unit cell,the relationship between pathological characteristics and HPV positivity in patients with different cervical lesions was analyzed,and the HPV viral load in patients with different pathological subtypes was compared,the correlation between HPV viral load and cervical intraepithelial neosplasi(CIN)grading was analyzed.Results The results of quantitative detection of HPV nucleic acid showed that there were 61 cases of HPV positive in 82 patients with cervical lesions,including 19 cases of low-grade squamous intraepithelial lesion(LSIL),18 cases of high-grade squamous intraepithelial lesion(HSIL)and 24 cases of cervical cancer.There were significant differences in the proportion of HPV positive in patients with different pathological types and parametrial invasion(P<0.05);there was no significant difference in the proportion of HPV positive in patients with different ages,maximum tumor diameter and lymph node metastasis.The HPV viral load increased with the increase of cervical lesions,and the difference was statistically significant(P<0.05),the HPV viral load of HSIL and cervical cancer patients was higher than that of LSIL patients,and that of cervical cancer patients was higher than that of HSIL patients,and the differences were statistically significant(P<0.05).Pearson correlation analysis showed that HPV viral load was positively correlated with CIN grade(r=0.767,P<0.05).Conclusion HPV nucleic acid quantitative detection technology can accurately analyze the HPV infection status and viral load in patients with cervical lesions,which is helpful for clinical diagnosis of the degree of cervical lesions and the risk of cancer in the examiners.
