摘要
目的探讨秦淮区1570例1~3月龄婴儿心脏彩色多普勒超声筛查情况。方法选取2019年1月至2020年12月秦淮区妇幼保健所参与常规体检的1570例1~3月龄婴儿作为研究对象。所有婴儿均进行心脏彩色多普勒超声检测,记录先天性心脏病筛查情况、先天性心脏病类型分布及卵圆孔未闭发生率,分析不同月龄婴儿先天性心脏病分布情况。结果1570例1~3月龄婴儿筛查出先天性心脏病患儿380例,占比24.20%;1~3月龄女婴先天性心脏病发生率为29.54%,高于男婴的19.67%,差异有统计学意义(P<0.05)。380例先天性心脏病中,房间隔缺损359例,占比94.47%;室间隔缺损17例,占比4.47%;肺动脉瓣狭窄2例,占比0.53%;动脉导管未闭2例,占比0.53%。10例0~1个月婴儿中,先天性心脏病患儿3例,占比30.00%;1407例>1~2个月婴儿中,先天性心脏病患儿358例,占比25.44%;153例>2~3个月婴儿中,先天性心脏病患儿19例,占比12.42%。1570例1~3月龄婴儿中,卵圆孔未闭患儿947例,卵圆孔未闭发生率为60.32%。结论秦淮区1~3月龄婴儿先天性心脏病检出率较高,女婴、>1~2个月婴儿群体,临床需增强先天性心脏病筛查工作,以便于早确诊、早治疗,并督促卵圆孔未闭婴儿定期复查,降低相关并发症发生风险。
Objective To investigate the status of cardiac color Doppler ultrasound screening in 1570 infants aged 1-3 months in Qinhuai district.Methods 1570 infants aged 1-3 months who participated in routine physical examination in Qinhuai District Maternal and Child Health Care Center from January 2019 to December 2020 were selected as the research subjects.All infants who underwent physical examination in our hospital were examined by cardiac color Doppler ultrasound.The screening of congenital heart disease,the distribution of congenital heart disease types and the incidence of patent foramen ovale were recorded,and the distribution of congenital heart disease in infants with different months of age was analyzed.Results Among the 1570 infants aged 1-3 months,380 cases of congenital heart disease were screened,accounting for 24.20%;the incidence of congenital heart disease in the female infants was 29.54%,which was higher than 19.67%in the male infants,and the difference was statistically significant(P<0.05).Among 380 cases of congenital heart disease,359 cases were atrial septal defect,accounting for 94.47%;17 cases of ventricular septal defect,accounting for 4.47%;2 cases of pulmonary valve stenosis,accounting for 0.53%;there were 2 cases of patent ductus arteriosus,accounting for 0.53%.Among the 10 infants aged 0-1 month,3 cases were congenital heart disease,accounting for 30.00%;among 1407 infants>1-2 months old,358 cases were congenital heart disease,accounting for 25.44%;among 153 infants>2-3 months,19 cases were congenital heart disease,accounting for 12.42%.Among the 1570 infants aged 1-3 months,there were 947 infants with patent foramen ovale,with patent foramen ovale incidence of 60.32%.Conclusion The detection rate of congenital heart disease in infants aged 1-3 months in Qinhuai District is high,and female infants and infants aged>1-2 months should be strengthened to screen for congenital heart disease in clinical practice,so as to facilitate early diagnosis and early treatment,infants with patent foramen ovale should be regularly reviewed to reduce the risk of related complications.
