重组人碱性成纤维细胞生长因子对小面积深Ⅱ度烧伤患者创面愈合率及远期瘢痕的影响

摘要 目的探究重组人碱性成纤维细胞生长因子(rh-bFGF)对小面积深Ⅱ度烧伤患者创面愈合率及远期瘢痕的影响。方法选取2018年6月至2021年3月本院烧伤科收治的100例小面积深Ⅱ度烧伤患者作为研究对象,随机分为常规组与实验组,每组50例。常规组给予创口常规换药治疗,实验组在常规组基础上联合rh-bFGF治疗,比较两组创面愈合率、创面50%愈合时间、完全愈合时间及瘢痕生长情况。结果两组创面愈合率组间、时间、交互比较差异有统计学意义(P<0.05);两组治疗后各时间点创面愈合率均高于前一时间点,且实验组低于常规组,差异有统计学意义(P<0.05)。实验组创面50%愈合时间及完全愈合时间均明显短于常规组,差异有统计学意义(P<0.05)。治疗3、6个月后,实验组瘢痕色泽、厚度、血管分布、柔软度评分及总分均低于常规组,且两组治疗6个月后瘢痕色泽、厚度、血管分布、柔软度评分及总分均低于治疗后3个月,差异有统计学意义(P<0.05)。结论rh-bFGF治疗小面积深Ⅱ度烧伤可提高创面愈合率,缩短愈合时间并减少远期瘢痕的形成,近、远期疗效均较好,值得临床推广应用。 Objective To observe the effect of recombinant human basic fibroblast growth factor(RH-bFGF)on wound healing rate and longterm scar in patients with small area deepⅡdegree burn.Methods A total of 100 patients with small area deepⅡdegree burn admitted to the treatment area of Burn department of our hospital from June 2018 to March 2021 were selected as the research subjects,and they were randomly divided into the conventional group and the experimental group,with 50 cases in each group.The conventional group was given conventional wound dressing change treatment,and the experimental group was given RH-bFGF treatment on the basis of conventional group,the wound healing rate,50%healing time,complete healing time and scar growth were compared between the two groups.Results There were significant differences in wound healing rate between the two groups of group,time points and interaction(P<0.05).The wound healing rates of the two groups at each time point after treatment were higher than those at the previous time point,and the experimental group was lower than the conventional group,the differences were statistically significant(P<0.05).The 50%healing time and complete healing time of the wound in the experimental group were significantly shorter than those in the conventional group,and the differences were statistically significant(P<0.05).After 3 and 6 months of treatment,the scar color,thickness,vascular distribution,softness score and total score in the experimental group were lower than those in the conventional group,and the scar color,thickness,vascular distribution,softness score and total score of the two groups after 6 months of treatment were lower than those after 3 months of treatment,the differences were statistically significant(P<0.05).Conclusion RH-bFGF can improve the wound healing rate,shorten the healing time and reduce the formation of the forward scar in the treatment of small area deepⅡdegree burn,both short-term and long-term effects are good,which is worthy of clinical promotion and application.
