腰椎术后1年非计划再入院原因及危险因素

Reasons and risk factors of unplanned readmission within 1 year after lumbar surgery

目的探讨腰椎术后1年非计划再入院原因及相关危险因素。方法选取2015年1月至2020年12月因腰椎退行性疾病入院行手术治疗的855例患者,分为再入院组(130例)和非再入院组(725例)。对两组之间临床资料进行比较,分析不同时间段非计划再入院危险因素;对再入院组入院的原因进行分析。结果腰椎术后1年非计划再入院发生率15.20%。与非再入院组相比,再入院组患者年龄更大[(58.72±12.42)岁vs(47.28±14.39)岁,P<0.05],凝血异常更多(16.92%vs 8.97%,P<0.05),既往脊柱手术史比例更高(16.15%vs 8.00%,P<0.05),融合手术数量(P<0.05)及手术节段更多(P<0.05),手术时间更长[(210.86±99.32)min vs(189.53±89.51)min,P<0.05],合并症数量更多(P<0.05),脑脊液漏(P<0.05)和骨质疏松患者更多(P<0.05)。结论不同时间段非计划再入院危险因素并不相同,主要取决于患者自身因素和手术相关因素。

Objective To investigate the cause and related risk factors of unplanned readmission within 1 year after lumbar surgery.Methods Patients who diagnosed with lumbar degenerative diseases between January 2015 and December 2020 were included in this study and divided into a non-readmission group(n=725)and a readmission group(n=130).The clinical data between the 2 groups were compared and risk factors were analysed.The reasons of the readmission group were analyzed.Results The incidence of unplanned readmission within 1 year after lumbar surgery was 15.20%.Compared with the non-readmission group,readmission group showed elder age[(58.72±12.42)years vs(47.28±14.39)years,P<0.05],more coagulation abnormalities(16.92%vs 8.97%,P<0.05),previous spinal surgery history(16.15%vs 8.00%,P<0.05),number of fusion operations(P<0.05),and operative segments(P<0.05),longer operation time[(210.86±99.32)min vs(189.53±89.51)min,P<0.05],more complications(P<0.05),cerebrospinal fluid leak(P<0.05)and osteoporosis(P<0.05).Conclusions The risk factors of unplanned readmission in different time periods are different and depend on the patient s own factors and surgical related factors.
