不同睾酮水平的勃起功能障碍患者中医治疗效果分析

Effects of traditional Chinese medicine on erectile dysfunction patients with different testosterone levels

目的:分析不同睾酮水平的勃起功能障碍患者中医治疗效果。方法:选取2022年1—12月于枣庄市立医院男科门诊就诊的112例勃起功能障碍患者,根据临床诊治经验,将符合条件的勃起功能障碍患者根据血清总睾酮水平分为两组,其中血清总睾酮水平≤300 ng/dL为低水平组(83例),血清总睾酮水平≥400 ng/dL为高水平组(29例)。高水平组患者给予滋补肾阴辅以补阳的治疗,低水平组患者给予温补肾阳辅以滋阴治疗。两组疗程均为2~4周。比较两组患者治疗前、后的睾酮水平、勃起功能国际问卷评分以及治疗效果。结果:治疗后低水平组患者的血清总睾酮水平高于治疗前,差异有统计学意义(P<0.01)。高水平组患者治疗后的血清总睾酮水平低于治疗前,差异有统计学意义(P<0.01)。两组患者治疗后的勃起功能障碍症状评分(IIEF-5)均高于治疗前,差异有统计学意义(P<0.05)。低水平组患者的临床治疗总有效率为92.77%,高水平组临床治疗总有效率为86.21%。结论:根据血清总睾酮水平选择温补肾阳或滋补肾阴治疗患者勃起功能障碍有明显疗效,故血清总睾酮水平可以作为勃起功能障碍患者采取补阳或滋阴治疗的一个依据。

Objectives To explore the therapeutic strategies of traditional Chinese medicine of different testosterone levels in erectile dysfunction patients.Methods A total of 112 patients with erectile dysfunction were selected from the male outpatient department of Zaozhuang Municipal Hospital from January 2022 to December 2022.According to clinical diagnosis and treatment experience,eligible patients with erectile dysfunction were divided into two groups according to serum total testosterone level.The serum total testosterone level≤300 ng/dL was the low level group(83 cases),and the serum total testosterone level≥400 ng/dL was the high level group(29 cases).The patients in the high level group were given the treatment of enrich Yin and tonify Yang,and the patients in the low level group were given the treatment of tonify Yang.The treatment course of both groups was 2 to 4 weeks.The testosterone level,erectile function international questionnaire score before and after treatment and therapeutic effect of the two groups were compared.Results The serum total testosterone level in the low level group was higher after treatment than before treatment,and the difference was statistically significant(P<0.01).The serum total testosterone level in the high level group after treatment was lower than before treatment,and the difference was statistically significant(P<0.01).The score of erectile dysfunction symptom(IIEF-5)after treatment was higher than that before treatment,and the difference was statistically significant(P<0.05).The total effective rate was 92.77%in the low level group and 86.21%in the high level group.Conclusions The treatment of warm and tonifying kidney Yang or enrich kidney Yin according to the serum total testosterone level has obvious effect on the improvement of erectile function.The serum total testosterone level can be used as a basis for tonify Yang or enrich Yin in patients with erectile dysfunction.
