胫骨横向骨搬移技术在糖尿病足治疗中的临床疗效

Clinical efficacy of tibial transverse bone transfer technique in the treatment of diabetes foot

目的探讨胫骨横向骨搬移技术在糖尿病足治疗中的临床疗效。方法选取2020年1月至2021年2月本院收治的20例糖尿病足患者作为研究对象,以随机数字表法分为对照组与实验组,每组10例。对照组采用开放性清创术治疗,实验组采用胫骨横向骨搬移技术治疗,比较两组临床疗效、治疗前后患肢溃疡面积、皮肤温度、疼痛程度及生命质量。结果两组治疗总有效率比较差异无统计学意义;治疗后,两组患肢溃疡面积均小于治疗前,视觉模拟评分法(VAS)评分均低于治疗前,皮肤温度均高于治疗前,且实验组患肢溃疡面积小于对照组,VAS评分低于对照组,皮肤温度高于对照组,差异有统计学意义(P<0.05);治疗后,实验组社会功能、躯体功能、物质生活、心理功能评分均高于对照组,差异有统计学意义(P<0.05)。结论胫骨横向骨搬移技术治疗糖尿病足可明显改善患者皮肤温度、疼痛程度,缩小患肢溃疡面积,提高患者生命质量,值得临床推广应用。

Objective To investigate the clinical efficacy of transverse tibial bone transfer technique in the treatment of diabetic foot.Methods 20 patients with diabetes foot admitted to our hospital from January 2020 to February 2021 were selected as the research subjects,and they were divided into the control group and the experimental group according to random number table method,with 10 case in each group.The control group was treated with open debridement,and the experimental group was treated with transverse tibial bone transfer technique,the clinical efficacy,ulcer area of affected limb,skin temperature,pain degree and quality of life before and after treatment were compared between the two groups.Results The total effective rate in the experimental group was significantly higher than that in the control group,the differences were statistically significant(P<0.05);after treatment,the ulcer area of the affected limb in the two groups was smaller than that before treatment,the pain degree was lower than that before treatment,and the skin temperature was higher than that before treatment,and the ulcer area of the affected limb in the experimental group was smaller than that in the control group,the pain degree scores was lower than that in the control group,and the skin temperature was higher than that in the control group,the differences were statistically significant(P<0.05);after treatment,the scores of social function,physical function,material life and psychological function in the experimental group were higher than those in the control group,the differences were statistically significant(P<0.05).Conclusion Tibial transverse bone transfer technique in the treatment of diabetes foot can significantly improve the skin temperature and pain degree of patients,reduce the ulcer area of affected limbs,and improve the quality of life of patients,which is worthy of clinical promotion and application.
