C反应蛋白与白蛋白比值在诊断结核性胸腔积液中的应用价值

The value of c reactive protein and albumin ratio in the diagnosis of the application value of tuberculous pleural effusion

目的探讨C反应蛋白与白蛋白比值(CAR)在诊断结核性胸腔积液中的应用价值。方法回顾性分析2018年1月至2020年12月本院收治的70例胸腔积液患者的临床资料,按照积液性质将其分为结核性胸腔积液组(n=44)与恶性胸腔积液组(n=26),收集患者外周血C反应蛋白、白蛋白及血常规、胸腔积液各项指标,通过ROC分析CAR在诊断结核性胸腔积液中的应用价值。结果结核性胸腔积液组与恶性胸腔积液组的CAR值比较差异有统计学意义(P<0.05);CAR诊断结核性胸腔积液的ROC曲线下面积为0.783,当截断值在0.34时,灵敏度为84.10%,特异度为61.50%。结论结核性胸腔积液与恶性胸腔积液的CAR值有明显的差异,CAR对诊断结核性胸腔积液具有一定的应用价值。

Objective To explore the application value of c-reaction protein and albumin ratio in the diagnosis of tuberculous pleural effusion.Methods The clinical data of 70 patients with pleural effusion admitted to our hospital from January 2018 to December 2020 were retrospectively analyzed,and they were divided into tuberculous pleural effusion group(n=44)and malignant pleural effusion group(n=26)according to the nature of the effusion,and collected all patients'peripheral blood C reaction protein,albumin and blood routine,and the various index of chest fluid,and analyzed the application value of car in diagnosing tuberous pleural fluid.Results There was statistically significant difference in CAR values between the tuberculous pleural effusion group and the malignant pleural effusion group(P<0.05).The roc curve of car diagnosis of tuberculous chest fluid was 0.783.when the truncyit was 0.34,the sensitivity was 84.10%,and the specificity was 61.50%.Conclusion There is significant difference in the car value of the tuberculous pleural effusion and malignant pleural effusion,and the application value of the diagnosis of tuberculous pleural effusion.
