消化内镜下注射去甲肾上腺素结合钛夹对非静脉曲张上消化道出血患者止血有效率和机体应激因子水平的影响

Effect of digestive endoscopic injection of norepinephrine combined with titanium clips on hemostatic efficacy and body stress factor levels in patients with non-varose upper gastrointestinal bleeding

目的探讨消化内镜下注射去甲肾上腺素结合钛夹对非静脉曲张上消化道出血患者止血有效率和机体应激因子水平的影响。方法选取2018年5月至2020年5月本院收治的60例非静脉曲张上消化道出血患者作为研究对象,随机分为观察组与对照组,每组30例。对照组采用消化内镜下注射去甲肾上腺素治疗,观察组在对照组治疗基础上配合钛夹止血,比较两组应激反应指标、止血效果有效率及不良反应发生情况。结果观察组皮质醇(Cor)、生长激素(GH)、抗利尿激素(ADH)水平均低于对照组,差异有统计学意义(P<0.05);观察组治疗总有效率(90.00%)高于对照组(66.67%),差异有统计学意义(P<0.05);观察组不良反应发生率(16.67%)低于对照组(40.00%),差异有统计学意义(P<0.05)。结论针对非静脉曲张上消化道出血患者,予以消化内镜下注射去甲肾上腺素结合钛夹作治疗疗效显著,能提高患者止血效果,降低其机体应激反应,患者不良反应少,安全性较高,值得临床推广应用。

Objective To investigate the effects of digestive endoscopic injection of noradrenaline combined with titanium clip on hemostatic response rate and body stress factor levels in patients with non-varose upper gastrointestinal bleeding.Methods 60 patients with non-varices with upper gastrointestinal bleeding admitted to our hospital from May 2018 to May 2020 were selected as the research subjects,and they were randomly divided into observation group and control group,with 30 cases in each group.The control group was treated with injection of norepinephrine under digestive endoscopic,and the observation group was treated with titanium clip on the basis of the control group,the stress response indexes,hemostatic efficiency and the occurrence of adverse reactions were compared between the two groups.Results The levels of cortisol(Cor),growth hormone(GH),and antidiuretic hormone(ADH)in observation group were lower than those in control group,the differences were statistically significant(P<0.05);the total effective rate of observation group(90.00%)was higher than that of control group(66.67%),the difference was statistically significant(P<0.05);the incidence of adverse reactions in the observation group(16.67%)was lower than the control group(40.00%),and the difference was statistically significant(P<0.05).Conclusion For patients with non-varicose upper gastrointestinal bleeding,norepinephrine injection under digestive endoscope combined with titanium clip has significant therapeutic effect,which can improve the hemostatic effect of patients and reduce the stress response of the body,with fewer adverse reactions and high safety,worthy of clinical promotion and application.
