急性阑尾炎及伴发并发症患者的CT影像表现及其诊断价值

CT imaging manifestations and diagnostic value of acute appendicitis and its accompanying complications

目的探讨急性阑尾炎及伴发并发症患者的CT影像表现及诊断效果。方法选取2019年6月至2022年6月邳州市中医院收治的120例阑尾炎患者作为研究对象,所有患者均行CT检查,以手术病理切片为金标准,观察CT检查影像学特征、诊断符合率、特异度与灵敏度。结果120例阑尾炎患者中,阑尾肿胀55例,管壁增厚33例,阑尾粪石16例,阑尾周围可见炎性渗出35例,部分患者包含阑尾穿孔、阑尾周围脓肿、局限性腹膜炎等表现。手术病理切片结果显示,120例阑尾炎患者中,急性阑尾炎患者占比为70.83%;CT检查结果显示,急性阑尾炎患者占比68.33%;CT检查诊断符合率为95.83%、特异度为94.29%、灵敏度为96.47%。CT检查与金标准诊断急性阑尾炎伴发其并发症准确率比较差异无统计学意义。结论CT检查在急性阑尾炎伴发并发症诊断中具有较高应用价值,可准确鉴别患者病情,值得临床推广。

Objective To investigate the CT imaging manifestations and diagnostic effects of acute appendicitis and its accompanying complications.Methods 120 patients with appendicitis admitted to Pizhou Hospital of Traditional Chinese Medicine from June 2019 to June 2022 were selected as the study subjects,all patients were examined by CT,and the imaging characteristics,diagnostic coincidence rate,specificity and sensitivity of CT were observed with the surgical pathology section as the gold standard.Results Among the 120 patients with appendicitis,55 had swelling of the appendix,33 had thickening of the tube wall,16 had fecal calculus of the appendix,and 35 had inflammatory exudation around the appendix,some patients included perforation of the appendix,abscess around the appendix,and localized peritonitis.The results of surgical pathology showed that among 120 patients with appendicitis,the proportion of acute appendicitis was 70.83%;the results of CT examination showed that the proportion of acute appendicitis was 68.33%;the diagnostic accuracy,specificity and sensitivity of CT were 95.83%,94.29%and 96.47%,respectively.There was no statistically significant difference in the accuracy of CT examination and gold standard in the diagnosis of acute appendicitis with complications.Conclusion In the diagnosis of acute appendicitis and its accompanying complications,CT examination has high application value,can accurately identify the patient's condition,and is worthy of clinical promotion.
