宫腔镜电切术治疗黏膜下子宫肌瘤患者的临床疗效

Clinical efficacy of hysteroscopic electroresection in treatment of patients with submucosal uterine fibroids

目的探讨宫腔镜电切术治疗黏膜下子宫肌瘤患者的临床疗效。方法选取2018年1月至2020年12月本院收治的70例黏膜下子宫肌瘤患者作为研究对象,按照手术方法分为观察组与对照组,每组35例。观察组采用宫腔镜电切术治疗,对照组采用开腹手术治疗,比较两组手术及术后情况、术后并发症发生情况及视觉模拟评分法(VAS)评分。结果观察组手术时间、术后排气时间、下床活动时间、住院时间均短于对照组,术中出血量少于对照组,差异有统计学意义(P<0.05);观察组术后并发症发生率(5.71%)低于对照组(25.71%),差异有统计学意义(P<0.05);两组VAS评分组间、时间、交互比较差异有统计学意义(P<0.05);组内比较:术后4、6周,两组VAS评分均低于术后2周,且术后6周低于术后4周,差异有统计学意义(P<0.05);组间比较:术后2、4、6周,观察组VAS评分均低于对照组,差异有统计学意义(P<0.05)。结论采用宫腔镜电切术治疗黏膜下子宫肌瘤效果确切,可降低术后并发症发生风险,总体效果优于常规开腹手术。

Objective To investigate the clinical efficacy of hysteroscopic electroresection in treatment of patients with submucosal uterine myoma.Methods 70 patients with submucosal uterine fibroids from January 2018 to December 2020 were selected as the research subjects,and they were divided into the observation group and the control group according to surgical methods,with 35 cases in each group.The observation group was treated with hysteroscopic electrotomy,while the control group was treated with open surgery,the operation and postoperative conditions,postoperative complications and visual analog scale(VAS)scores were compared between the two groups.Results The operation time,postoperative exhaust time,getting out of bed time and hospitalization time in the observation group were shorter than those in the control group,and the intraoperative blood loss was less than that in the control group,the differences were statistically significant(P<0.05).The incidence of postoperative complications in observation group(5.71%)was lower than that in control group(25.71%),and the difference was statistically significant(P<0.05).There were significant differences in VAS inter-group,time and interaction between the two groups(P<0.05);intra-group comparison:4,6 weeks after surgery,the VAS scores of the two groups were lower than 2 weeks after surgery,and 6 weeks after surgery was lower than 4 weeks after surgery,the difference was statistically significant(P<0.05);inter-group comparison:at 2,4 and 6 weeks after surgery,the VAS scores in the observation group were lower than those in the control group,the differences were statistically significant(P<0.05).Conclusion Hysteroscopic electroresection is effective in the treatment of submucosal uterine fibroids,and can reduce the risk of postoperative complications,the overall effect is better than that of conventional laparotomy.
