比阿培南联合莫西沙星对重症肺炎患者免疫功能及炎症指标的影响

Effect of biapenem combined with moxifloxacin on immune function and inflammatory indexes in patients with severe pneumonia

摘要 目的探讨比阿培南联合莫西沙星对重症肺炎患者免疫功能及炎症指标的影响。方法选取2018年1月至2020年1月本院收治的88例重症肺炎患者作为研究对象,按照随机数字表法分为观察组与对照组,各44例。对照组给予比阿培南治疗,观察组在对照组基础上联合莫西沙星治疗,比较两组临床疗效、免疫功能、血气指标[动脉血氧分压(PO_(2))、氧合指数(PO_(2)/FiO_(2))、pH值]、炎症指标[降钙素原(PCT)、白细胞计数(WBC)、超敏C反应蛋白(hs-CRP)]及不良反应发生情况。结果观察组治疗总有效率为93.18%,高于对照组的77.27%,差异有统计学意义(P<0.05)。治疗后,两组PO_(2)、PO_(2)/FiO_(2)、pH值水平均高于治疗前,且观察组高于对照组,差异有统计学意义(P<0.05)。治疗后,两组CD3+、CD4+水平均高于治疗前,CD8+水平均低于治疗前,且观察组CD3+、CD4+水平均高于对照组,CD8+水平低于对照组,差异有统计学意义(P<0.05)。治疗后,两组PCT、WBC、hs-CRP水平均低于治疗前,且观察组低于对照组,差异有统计学意义(P<0.05)。观察组不良反应发生率为6.82%,对照组为13.64%,组间比较差异无统计学意义。结论比阿培南联合莫西沙星治疗重症肺炎疗效显著,可有效改善患者免疫功能和血气指标,减轻炎症反应。 Objective To investigate the effect of biapenem combined with moxifloxacin on immune function and inflammatory indexes in pa-tients with severe pneumonia.Methods A total of 88 patients with severe pneumonia admitted to our hospital from January 2018 to January 2020 were selected as the research subjects,they were divided into the observation group and the control group according to random number table method,with 44 cases in each group.The control group was treated with biapenem,while the observation group was treated with moxifloxacin on the basis of the control group,the clinical efficacy,immune function,blood gas indexes(partial pressure of oxygen[PO_(2)],partial pressure of oxygen/inspired oxy-gen concentration[PO_(2)/FiO_(2)],pH value),inflammatory indexes(procalcitonin[PCT],white blood cell count[WBC]and high-sensitivity C-reactive protein[hs-CRP])and the incidence rates of adverse reactions were compared between the two groups.Results The total effective rate in the obser-vation group was 93.18%,which was higher than 77.27%in the control group,and the difference was statistically significant(P<0.05).After treat-ment,the levels of PO_(2),PO_(2)/FiO_(2) and pH value of the two groups were higher than those before treatment,and the observation group was higher than the control group,the differences were statistically significant(P<0.05).After treatment,the levels of CD3+and CD4+of the two groups were higher than those before treatment,and the level of CD8+was lower than that before treatment,and the levels of CD3+and CD4+in the observation group were higher than those in the control group,and the level of CD8+was lower than that in the control group,the differences were statistically signifi-cant(P<0.05).After treatment,the levels of PCT,WBC and hs-CRP of the two groups were lower than those before treatment,and the observation group was lower than the control group,the differences were statistically significant(P<0.05).The incidence rates of adverse reactions in the obser-vation group was 6.82%,and 13.64%in the control group,and there was no statistically significant difference between groups.Conclusion Biapen-em combined with moxifloxacin is effective in the treatment of severe pneumonia,which can effectively improve the immune function and blood gas indexes of patients,and reduce inflammatory reduce.
