接受IVF-ET助孕子宫内膜息肉不孕患者SCL-90评分状况与其应对方式的相关性

Correlation between Symptom Checklist-90 score and coping style of infertile patients with endometrial polyp receiving in vitro fertilization-embryo transfer

目的调查接受体外受精-胚胎移植(IVF-ET)助孕子宫内膜息肉不孕患者症状自评量表(SCL-90)评分状况,并探讨其与患者应对方式的相关性。方法以便利抽样法选取2020年10月至2022年10月南阳市第一人民医院生殖医学科收治的152例接受IVF-ET助孕子宫内膜息肉不孕患者作为观察组,另选取1388例健康人群作为对照组,采用一般资料调查表、SCL-90量表、女性不孕症患者羞辱感量表(ISS)、生育压力量表(FPI)、特质应对方式(TCSQ)、领悟社会支持量表(PSSS)、一般自我效能量表(GSES)进行横断面调查。采用多元线性回归分析SCL-90评分影响因素,并采用Pearson相关性分析SCL-90评分与应对方式之间关系。结果观察组患者的焦虑、抑郁、恐怖、精神病性评分及总分明显高于对照组,偏执、人际敏感评分明显低于对照组,差异均有统计学意义(P<0.05);观察组中不同文化程度、月收入水平、FPI评分、ISS评分、自我效能感、社会支持水平患者的SCL-90评分比较,差异均具有统计学意义(P<0.05);经多元线性回归分析结果显示,月收入水平、FPI评分、ISS评分、自我效能感、社会支持水平均是接受IVF-ET助孕子宫内膜息肉不孕患者SCL-90评分的影响因素(P<0.05);经Pearson相关性分析结果显示,积极应对与SCL-90评分呈正相关(r=0.436,P<0.001),消极应对与SCL-90评分呈负相关(r=-0.410,P<0.001)。结论接受IVF-ET助孕子宫内膜息肉不孕患者心理状态普遍较差,主要与月收入水平、FPI评分、ISS评分、自我效能感、社会支持水平等因素有关,且与应对方式存在密切关联,治疗时应采取相应措施提高患者应对疾病能力,通过提高心理弹性水平缓解其负性情绪,进而提高其积极应对方式。

Objective To investigate the score of Symptom Checklist-90(SCL-90)in infertile patients with endometrial polyp receiving in vitro fertilization-embryo transfer(IVF-ET),and to explore its correlation with patients'coping style.Methods A total of 152 infertile patients with endometrial polyp who received IVF-ET from October 2020 to October 2022 in the Department of Reproductive Medicine,the First People's Hospital of Nanyang City were selected as the observation group by convenience sampling method,and another 1388 healthy people were selected as the control group.The cross-sectional survey was conducted by using general information questionnaire,SCL-90 scale,Infertility Stigma Scale(ISS),Fertility Pressure Scale(FPI),Trait Coping Style Questionnaire(TCSQ),Perceived Social Support Scale(PSSS),and General Self-Efficacy Scale(GSES).Multiple linear regression was used to analyze the influencing factors of SCL-90 scores,and Pearson correlation was used to analyze the relationship between SCL-90 scores and coping styles.Results Anxiety,depression,terror,and psychoticism scores,and total scores in the observation group were significantly higher than those in the control group,and the paranoia and interpersonal sensitivity scores were significantly lower than those in the control group,with statistically significant differences(P<0.05).There were statistically significant differences in SCL-90 scores among patients with different levels of education,monthly income,FPI score,ISS score,self-efficacy,and social support(P<0.05).Multiple linear regression analysis showed that monthly income level,FPI score,ISS score,self-efficacy,and social support level were the influencing factors of SCL-90 score(P<0.05).Pearson correlation analysis showed that positive coping was positively correlated with SCL-90 scores(r=0.436,P<0.001),while negative coping was negatively correlated with SCL-90 scores(r=-0.410,P<0.001).Conclusion Infertile patients with endometrial polyp who receive IVF-ET generally have poor psychological status,which is mainly related to factors such as monthly income level,FPI score,ISS score,self-efficacy,and social support level,and was closely associated with coping style.Appropriate measures should be taken during treatment to improve patients'ability to cope with the disease,alleviate their negative emotions by increasing their psychological resilience level,and thus improve their positive coping style.
