膀胱功能训练联合鼠神经生长因子治疗神经源性膀胱的效果及对患者排尿功能和预后的影响

摘要 目的探究膀胱功能训练联合鼠神经生长因子(mNGF)治疗神经源性膀胱的效果及对患者排尿功能和预后的影响。方法选取2019年6月至2021年10月本院收治的92例神经源性膀胱患者作为研究对象,随机分为对照组和研究组,每组46例。对照组采用膀胱功能训练治疗,研究组采用膀胱功能训练联合mNGF治疗,比较两组临床疗效、排尿情况、尿动力学水平、尿路感染情况。结果研究组治疗总有效率(89.13%)高于对照组(71.74%),差异有统计学意义(P<0.05)。治疗前,两组排尿量、排尿次数比较差异无统计学意义;治疗后,两组排尿量均大于治疗前,排尿次数均多于治疗前,且研究组排尿量大于对照组,排尿次数多于对照组,差异有统计学意义(P<0.05)。治疗前,两组膀胱容量、膀胱压力、尿残余量比较差异无统计学意义;治疗后,两组膀胱容量、膀胱压力均大于治疗前,尿残余量均小于治疗前,且研究组膀胱容量、膀胱压力均大于对照组,尿残余量小于对照组,差异有统计学意义(P<0.05)。观察组尿路感染阳性率(13.04%)低于对照组(32.61%),差异有统计学意义(P<0.05)。结论膀胱功能训练联合mNGF治疗神经源性膀胱效果显著,不仅可恢复患者膀胱排尿功能,改善排尿困难和尿潴留等症状,还可降低尿路感染发生率,值得临床推广应用。 Objective To explore the effect of bladder function training combined with rat nerve growth factor(mNGF)in the treatment of neurogenic bladder and its influence on urination function and prognosis of patients.Methods A total of 92 patients with neurogenic bladder admitted to our hospital from June 2019 to October 2021 were selected as the research subjects and they were randomly divided into control group and study group,with 46 cases in each group.The control group was treated with bladder function training,while the study group was treated with bladder function training combined with mNGF,the clinical efficacy,urination,urodynamic level and urinary tract infection were compared between the two groups.Results The total effective rate of study group(89.13%)was higher than that of control group(71.74%),and the difference was statistically significant(P<0.05).Before treatment,there was no significant difference in urination volume and urination frequency between the two groups;after treatment,the urination volume of the two groups was greater than that before treatment,and the urination frequency was more than that before treatment,the urination volume of the study group was greater than that of the control group,and the urination frequency was more than that of the control group,the differences were statistically significant(P<0.05).Before treatment,there was no significant difference in bladder volume,bladder pressure and urinary residual volume between the two groups;after treatment,the bladder volume and bladder pressure of the two groups were higher than before treatment,and the urinary residual volume was lower than before treatment.The bladder volume and bladder pressure of the study group were higher than those of the control group,and the urinary residual volume was lower than that of the control group,and the differences were statistically significant(P<0.05).The positive rate of urinary tract infection in the observation group(13.04%)was lower than that in the control group(32.61%),and the difference was statistically significant(P<0.05).Conclusion Bladder function training combined with rat nerve growth factor is effective in the treatment of neurogenic bladder,which can not only restore the bladder function,improve the symptoms of dysuria and urinary retention,but also reduce the incidence of urinary tract infection,which is worthy of clinical application.
