单腔起搏器与双腔起搏器对缓慢性心律失常老年患者心功能及生命质量的影响

Effects of single-chamber pacemaker and dual-chamber pacemaker on cardiac function and quality of life in elderly patients with bradycardia and chronic arrhythmia

目的探讨单腔起搏器与双腔起搏器对缓慢性心律失常老年患者心功能及生命质量的影响。方法选取2018年5月至2020年5月本院收治的92例缓慢性心律失常老年患者作为研究对象,按照随机数字表法分为单腔起搏器组与双腔起搏器组,每组46例。单腔起搏器组置入单腔起搏器,双腔起搏器组置入双腔起搏器,比较两组心功能、生命质量、纽约心脏病协会(NYHA)分级及心力衰竭住院率。结果治疗后,双腔起搏器组左心室射血分数(LVEF)、健康调查简表(SF-36)评分分别为(59.08±5.13)%、(69.82±7.53)分,均高于单腔起搏器组的(52.37±4.61)%、(53.64±8.06)分,左心室舒张末期内径(LVEDD)为(49.37±4.10)mm,短于单腔起搏器组的(52.79±5.41)mm,NYHA分级优于单腔起搏器组,心力衰竭住院率(10.87%)低于单腔起搏器组(30.43%),差异有统计学意义(P<0.05)。结论双腔起搏器治疗缓慢性心律失常效果显著,能减轻对患者心功能的影响及心力衰竭严重程度,降低住院率,改善患者生命质量及预后,值得临床推广应用。

Objective To investigate the effects of single-chamber pacemaker and dualchamber pacemaker on cardiac function and quality of life in elderly patients with bradyarrhythmia and chronic arrhythmia.Methods 92 elderly patients with bradycardia and chronic arrhythmia admitted to our hospital from May 2018 to May 2020 were selected as the study subjects,and they were divided into single-chamber pacemaker group and double-chamber pacemaker group according to the random number table method,with 46 cases in each group.The single-chamber pacemaker group was implanted with single-chamber pacemaker,and the double-chamber pacemaker group was implanted with double-chamber pacemaker,the cardiac function,quality of life,New York Heart Association(NYHA)classification and hospitalization rate of heart failure were compared between the two groups.Results After treatment,the left ventricular ejection fraction(LVEF)and the MOS 36item short form health survey(SF-36)scores in the double-chamber pacemaker group were(59.08±5.13)%and(69.82±7.53)scores respectively,which were higher than(52.37±4.61)%and(53.64±8.06)scores in the single-chamber pacemaker group,and the left ventricular end-diastolic diameter(LVEDD)was(49.37±4.10)mm,which was lower than(52.79±5.41)mm in the single-chamber pacemaker group,NYHA grading was better than that of single-chamber pacemaker group,and the hospitalization rate of heart failure(10.87%)was lower than that of single-chamber pacemaker group(30.43%),the differences were statistically significant(P<0.05).Conclusion Double-chamber pacemaker has significant effect in the treatment of bradyarrhythmia,it can reduce the impact on cardiac function and the severity of heart failure,reduce the hospitalization rate,improve the quality of life and prognosis of patients,and is worthy of clinical application.
