CT与MRI在眼眶炎性假瘤的诊断效果及灵敏度分析

目的研究CT和MRI在眼眶炎性假瘤诊断中的应用价值及诊断灵敏度。方法选取2017年5月至2020年8月本院收诊的60例高度疑似眼眶炎性假瘤患者作为研究对象,均进行CT和MRI扫描,以病理检查结果为诊断依据,比较CT与MRI对眼眶炎性假瘤的诊断符合率、灵敏度和特异度,并分析眼眶炎性假瘤的影像学特点。结果MRI对眼眶炎性假瘤的诊断符合率为95.0%,灵敏度为95.83%,均高于CT(76.67%、77.08%),差异有统计学意义(P<0.05);MRI(91.67%)与CT的诊断特异度(75.0%)比较差异无统计学意义;60例患者中,肿块型22例(27只眼),泪腺炎型7例(12只眼),肌炎型26例(30只眼),为弥漫型5例(6只眼)。结论CT和MRI在眼眶炎性假瘤的诊断中应用价值高,不同分型的眼眶炎性假瘤具有不同的影像学特点,可为临床诊疗提供可靠依据,其中MRI的应用价值更高,而CT可作为初步诊断的方法应用。

Objective To study the value and sensitivity of CT and MRI in the diagnosis of orbital inflammatory pseudotumor.Methods 60 patients with highly suspected orbital inflammatory pseudotumor admitted in our hospital from May 2017 to August 2020 were selected as the research subjects,all of whom underwent CT and MRI scanning,based on the pathological examination results,the diagnostic accuracy,sensitivity and specificity of CT and MRI for orbital inflammatory pseudotumor were compared,and the imaging characteristics of orbital inflammatory pseudotumor were analyzed.Results The diagnostic coincidence rate of MRI for orbital inflammatory pseudotumor was 95.0%,and the sensitivity was 95.83%,which were higher than those of CT(76.67%,77.08%),the differences were statistically significant(P<0.05);there was no significant difference in diagnostic specificity between MRI(91.67%)and CT(75.0%);among the 60 patients,22 cases were mass type(27 eyes),7 cases were lacrimal gland inflammation type(12 eyes),26 cases were myositis type(30 eyes),and 5 cases were diffuse type(6 eyes).Conclusion CT and MRI have high application value in the diagnosis of orbital inflammatory pseudotumor,different types of orbital inflammatory pseudotumor have different imaging characteristics,which can provide reliable basis for clinical diagnosis and treatment.MRI has higher application value,and CT can be used as a preliminary diagnosis method.
