帕利哌酮与奥氮平治疗首发精神分裂症的临床疗效

Clinical efficacy of paliperidone and olanzapine in the treatment of first-episode schizophrenia

目的探究帕利哌酮与奥氮平治疗首发精神分裂症的临床疗效。方法选取2019年12月至2020年12月本院收治的90例首发精神分裂症患者作为研究对象,按照病历单双号分为两组,每组45例。对照组(单号)接受奥氮平治疗,观察组(双号)接受帕利哌酮治疗,比较两组临床指标、临床疗效、不良反应发生率。结果治疗前,两组胰岛素、血浆瘦素水平及体重指数(BMI)比较差异无统计学意义;观察组治疗前后胰岛素、血浆瘦素水平及BMI比较差异无统计学意义。治疗后,对照组胰岛素、血浆瘦素水平及BMI均高于治疗前,但观察低于对照组,差异有统计学意义(P<0.05)。观察组治疗总有效率为93.3%,高于对照组的68.9%,差异有统计学意义(P<0.05)。观察组不良反应发生率为6.7%,低于对照组的28.9%,差异有统计学意义(P<0.05)。结论帕利哌酮与奥氮平治疗首发精神分裂症均有一定效果,但帕利哌酮的疗效更理想,不会影响患者胰岛素、血浆瘦素等指标,且不良反应少,安全性高,值得临床推广应用。

Objective To explore the clinical efficacy of paliperidone and olanzapine in the treatment of firstepisode schizophrenia.Methods 90 patients with firstepisode schizophrenia admitted to our hospital from December 2019 to December 2020 were selected as the research subjects,and they were divided into two groups according to the odd-even medical records,with 45 cases in each group.The control group(single number)received olanzapine treatment,and the observation group(double number)received paliperidone treatment,the clinical indexes,clinical efficacy and incidence of adverse reactions were compared between the two groups.Results Before treatment,there were no significant differences in the levels insulin,plasma leptin and body mass index(BMI)between the two groups.There was no significant difference in the levels of insulin,plasma leptin and BMI in the observation group before and after treatment.After treatment,the levels of insulin,plasma leptin and BMI in the control group were higher than before treatment,but those in the observation group were lower than the control group,the differences were statistically significant(P<0.05).The total effective rate in the observation group was 93.3%,which was higher than 68.9%in the control group,and the difference was statistically significant(P<0.05).The incidence of adverse reactions in the observation group was 6.7%,which was lower than 28.9%in the control group,and the difference was statistically significant(P<0.05).Conclusion Both paliperidone and olanzapine have certain effects in the treatment of first-episode schizophrenia,but the efficacy of paliperidone is more ideal,does not affect insulin,plasma leptin and other indicators,and has fewer adverse reactions,high safety,which is worthy of clinical promotion and pplication.
