微型种植钉增强正畸支抗的稳定性及其影响因素分析

摘要 目的探讨微型种植钉增强正畸支抗的稳定性及其影响因素。方法回顾性分析2017年1月至2019年12月于临川区第一人民医院行正畸治疗的76例患者的临床资料。患者均植入微型种植钉,随访10个月,统计患者微型种植钉支抗稳定性情况,多因素Logistic回归分析微型种植钉增强正畸支抗稳定性的影响因素。结果76例正畸患者中微型种植钉支抗稳定性不良7例,占比9.21%;微型种植钉支抗稳定性良好69例,占比90.79%。稳定性不良患者与稳定性良好患者加力时机、加力方式、植入部位、微型种植钉长度及植入角度比较差异无统计学意义;稳定性不良患者与稳定性良好患者牙周炎史、下颌平面角及骨密度比较差异有统计学意义(P<0.05);Logistic回归分析结果显示,有牙周炎史、下颌平面角较高及骨密度低均为微型种植钉增强正畸支抗稳定性的影响因素(OR>1,P<0.05)。结论有牙周炎史、下颌平面角较高及骨密度低的正畸患者微型种植钉支抗稳定性不良,因此,临床可早期给予针对性的方案干预,以提高微型种植钉支抗的稳定性。 Objective To investigate the stability of orthodontic anchorage enhanced by mini-screw and its influencing factors.Methods The clinical data of 76 patients who underwent orthodontic treatment in the Fuzhou Lianchuan District First People's Hospital from January 2017 to December 2019 were retrospectively analyzed.All patients were implanted with mini-screw and followed up for 10 months,the stability of mini-screw anchorage was counted,and the influencing factors of mini-screw to enhance the stability of orthodontic anchorage were analyzed by multivariate Logistic regression.Results Among the 76 orthodontic patients,there were 7 cases of poor stability of mini-screw anchorage,accounting for 9.21%;69 cases of had good stability of mini-screw anchorage,accounting for 90.79%.There was no statistical significant difference between patients with poor stability and patients with good stability in loading time,loading method,implant site,mini-screw length and implant angle;there were statistical significant differences in periodontitis history,mandibular plane angle and bone mineral density between patients with poor stability and patients with good stability(P<0.05).Logistic regression analysis showed that history of periodontitis,high mandibular plane angle and low bone mineral density were the influencing factors of micro-screw enhancing orthodontic anchorage stability(OR>1,P<0.05).Conclusion Orthodontic patients with history of periodontitis,high mandibular plane angle and low bone density have poor stability of mini-screw,therefore,targeted program intervention can be given early in clinical practice to improve the stability of mini-screw.
