24h动态心电图检查法对冠心病性心律失常的诊断意义

The diagnostic significance of 24 h dynamic electrocardiogram examination on coronary heart disease arrhythmia

目的探讨冠心病性心律失常诊断中24h动态心电图的价值。方法选取2017年5月至2020年5月本院收治的100例冠心病性心律失常患者作为研究对象,分别接受常规心电图检查与24h动态心电图检查。比较两种检查方法心律失常阳性率;比较两种检查方式房性早搏、室上性早搏、房室传导阻滞检出率;比较两种检查方法成对房性早搏/二三联律、成对室性早搏/二三联律检出率。结果24 h动态心电图检出心律失常阳性率为73.00%,显著高于常规心电图的53.00%,差异有统计学意义(P<0.05);两种检查方法房性早搏、室上性早搏、房室传导阻滞在检出率比较差异无统计学意义;24 h动态心电图成对房性早搏/二三联律、成对室性早搏/二三联律检出率高于常规心电图,差异有统计学意义(P<0.05)。结论24 h动态心电图检查法诊断冠心病性心律失常检出率高于常规心电图,且对成对房性早搏/二三联律、成对室性早搏/二三联律等的诊断价值较高,值得临床推广运用。

Objective To explore the value of 24 h dynamic electrocardiogram in the diagnosis of coronary heart disease arrhythmia.Methods 100 patients with coronary heart disease arrhythmia admitted to our hospital from May 2017 to May 2020 were selected as the research subjects,and they were diagnosed by routine electrocardiography and dynamic electrocardiography.The positive rate of arrhythmia was compared between the two examinations;the detection rates of atrial premature beat,supraventricular premature beat and atrioventricular block were compared between the two examinations;the detection rates of atrial premature beats/bitrilogy and paired ventricular premature beats/bitrilogy were compared between the two examinations.Results The positive rate of arrhythmia detected in the dynamic electrocardiography was 73.00%,which was significantly higher than 53.00%in the routine electrocardiography,and the difference was statistically significant(P<0.05).There was no significant difference in the detection rate of atrial premature beats,supraventricular premature beats and atrioventricular block between the two examinations;the detection rates of paired atrial premature beats/two triple law and paired ventricular premature beats/two triple law in 24 h dynamic electrocardiogram were higher than those in routine electrocardiogram,and the differences were statistically significant(P<0.05).Conclusion The detection rate of 24 h dynamic electrocardiogram in the diagnosis of coronary heart disease arrhythmia is higher than that of conventional electrocardiogram,and the diagnostic value of paired atrial premature beat/two triple law,paired ventricular premature beat/two triple law is higher,which is worthy of clinical application.
