老年房颤患者抗凝药应用期间消化道出血的影响因素分析

Analysis of the influencing factors of gastrointestinal bleeding in elderly patients with atrial fibrillation during anticoagulant drug therapy

目的分析老年房颤患者抗凝药应用期间消化道出血的影响因素。方法回顾性分析2019年6月至2020年10月于本院接受抗凝药物治疗的72例老年房颤患者的临床资料,分析患者抗凝药应用期间消化道出血发生情况及其影响因素。结果72例老年房颤患者中,发生消化道出血10例,发生率为13.89%;未发生消化道出血62例,占比86.11%。发生消化道出血患者年龄、有吸烟史占比、有消化性溃疡史占比均高于未发生消化道出血患者,差异有统计学意义(P<0.05);经Logistic回归分析结果显示,高龄、有吸烟史、有消化性溃疡史为老年房颤患者抗凝药应用期间发生消化道出血的危险因素(OR>1,P<0.05)。结论老年房颤患者抗凝药应用期间存在一定消化道出血风险,而高龄、有吸烟史、有消化性溃疡史易导致老年房颤患者抗凝药应用期间出现消化道出血,临床应据此予以针对性干预措施,以改善患者预后。

Objective To analyze the influencing factors of gastrointestinal bleeding in elderly patients with atrial fibrillation during anticoagu-lant drug therapy.Methods The clinical data of 72 elderly patients with atrial fibrillation who were treated with anticoagulant drugs in our hospital from June 2019 to October 2020 were retrospectively analyzed,the occurrence of gastrointestinal bleeding and its influencing factors during the ap-plication of anticoagulants were analyzed.Results Among the 72 elderly patients with atrial fibrillation,10 cases had gastrointestinal bleeding,with the incidence of 13.89%;62 cases without gastrointestinal bleeding,accounting for 86.11%.The age,smoking history and peptic ulcer history of the patients with gastrointestinal bleeding were significantly higher than those without gastrointestinal bleeding,and the differences were statistically sig-nificant(P<0.05).Logistic regression analysis showed that old age,smoking history and peptic ulcer history were the risk factors for gastrointestinal bleeding during anticoagulant application in elderly patients with atrial fibrillation(OR>,P<0.05).Conclusion Elderly patients with atrial fibrilla-tion have certain risk of gastrointestinal bleeding during the application of anticoagulant drugs,while old age,smoking history and peptic ulcer histo-ry may lead to gastrointestinal bleeding during the application of anticoagulant drugs in elderly patients with atrial fibrillation,and clinical interven-tion measures should be taken accordingly to improve the prognosis of patients.
