化学发光法与酶联免疫吸附测定法在乙型肝炎病毒感染血清标志物检验中的应用效果比较

Comparison of application effect of chemiluminescence and enzyme-linked immunosorbent assay in detection of serum markers of hepatitis B virus infection

目的比较化学发光法与酶联免疫吸附测定法(ELISA)在乙型肝炎病毒(HBV)感染血清标志物检测中的应用效果。方法选取2018年6月至2021年3月本院收治的82例疑似乙型肝炎患者作为研究对象,所有患者均采取化学发光法及ELISA检测,以实时荧光定量聚合酶链式反应(PCR)法作为金标准,比较两种检测方法的诊断价值、阳性检出率、重复性。结果PCR共检出阳性53例,阴性29例;化学发光法阳性52例,阴性30例;ELISA检出阳性51例,阴性31例。化学发光法检测HBV感染血清标志物的灵敏度、特异度、阳性预测值、阴性预测值均明显高于ELISA,差异有统计学意义(P<0.05)。化学发光法HBeAb、HBeAg、HBsAg阳性检出率均明显高于ELISA,差异有统计学意义(P<0.05)。结论与ELISA比较,化学发光法检测HBV感染血清标志物效果更好,诊断价值、阳性检出率均较高,值得临床推广应用。

Objective To compare the effect of chemiluminescence and enzyme-linked immunosorbent assay(ELISA)in the detection of serum markers of hepatitis B virus(HBV)infection.Methods 82 patients of suspected hepatitis B admitted from June 2018 to March 2021 were selected as the research subjects,all patients took chemiluminescence and ELISA method,real-time quantitative polymerase chain reaction(PCR)method as the gold standard,and the diagnostic value,positive detection rate and reproducibility were compared between the two detection methods.Results A total of 53 cases were positive and 29 cases were negative by PCR,Chemiluminescence was positive in 52 cases and negative in 30 cases;ELISA was positive in 51 cases and negative in 31 cases.The sensitivity,specificity,positive predictive value and negative predictive value of the serum markers for HBV infection were significantly higher than those of ELISA,and the difference was statistically significant(P<0.05).The positive de-tection rates of HBeAb,HBeAg and HBsAg in the HBCs were significantly higher than those of ELISA and the differences were statistically signifi-cant(P<0.05).Conclusion Compared with ELISA,HBV infection serum marker detection has better effect,high diagnostic value,positive detec-tion rate,which is worthy of clinical application.
