摘要
目的探讨盆底超声在盆底重建术治疗盆底器官脱垂中的应用效果。方法选取2019年2月至2020年3月本院收治的86例盆底器官脱垂患者作为研究对象,所有患者于盆底重建术前及术后6个月行盆底超声检查。比较手术前后患者超声参数和静息状态(RS)、缩肛状态(CS)、最大Valsalva动作(MVA)下肛提肌裂口面积及尿道漏斗、尿道膨出发生率。结果术后6个月,患者膀胱颈移动度、尿道倾斜角-Val-salva动作、尿道旋转角-Valsalva动作均小于术前(P<0.05)。术后6个月,患者RS、CS、MVA状态下肛提肌裂口面积均小于术前(P<0.05)。术后6个月,患者尿道漏斗发生率为3.49%、尿道膨出发生率为4.65%,均低于术前的25.58%、18.60%(P<0.05)。结论盆底超声应用于盆底重建术治疗盆底器官脱垂中诊断价值较好,可明确超声参数、肛提肌裂口面积改善情况,诊断尿道漏斗及尿道膨出发生情况,可为临床评估预后提供影像学信息。
Objective To investigate the effect of pelvic floor ultrasound in pelvic floor reconstruction for pelvic floor organ prolapse.Methods A total of 86 patients with pelvic floor organ prolapse admitted to our hospital from February 2019 to March 2020 were selected as the research subjects,and all patients underwent pelvic floor ultrasound examination before and 6 months after pelvic floor reconstruction.The ultrasonic parameters,hia-tus area of levator anal muscle under resting state(RS),constriction state(CS),maximum Valsalva action(MVA),and incidence of urethral funnel and urethrocele were compared before and after operation.Results At 6 months after operation,the bladder neck mobility,urethral tilt angle-Valsal-va and urethral rotation angle-Valsalva were significantly lower than those before operation(P<0.05).At 6 months after operation,the area of leva-tor anal tear under RS,CS and MVA were smaller than those before operation(P<0.05).At 6 months after operation,the incidence of urethral in-fundibulum was 3.49%,and the incidence of urethral bulge was 4.65%,which were lower than 25.58%and 18.60%before operation(P<0.05).Conclusion Pelvic floor ultrasound has good diagnostic value in pelvic floor reconstruction for pelvic organ prolapse,can clarify the ultrasonic pa-rameters,the improvement of ultrasonic parameters the levator anal muscle cleft area,the diagnosis of urethral infundibulum and urethral bulge,and can provide imaging information for clinical evaluation of prognosis.
