腹部推拿的神经生物学机制研究进展

Research progress on the neurobiological mechanism of abdominalmassage

腹部推拿是祖国推拿医学中的重要组成部分,其临床疗效显著,可广泛运用于各科临床治疗,其治疗机理从中、西医角度皆有据可循。随着研究技术的不断改进,近年来对腹部推拿的发病机制的研究取得了较为明显的突破。本文总结了近二十年腹部推拿治疗疾病的神经生物学相关机制,归纳发现腹部推拿治疗以消化系统、神经系统及内分泌系统疾病为主,可通过调节神经递质水平(包括单胺类神经递质、肽类神经递质)、神经炎性反应、应激反应(包括下丘脑—垂体—肾上腺轴、氧化应激反应)、神经系统营养因子及基因蛋白表达(包括脑源性神经营养因子、胶质细胞源性神经营养因子,及其他神经系统基因蛋白表达)、神经元活性及代谢产物发挥治疗作用,体现了腹部推拿多靶点、双向性的作用特点。同时,本文还就腹部推拿的神经生物学机制研究存在的问题进行了讨论,提出目前该领域相关研究尚处于起步阶段,主要以实验研究为主,存在部分局限性,仍有待借鉴系统科学的研究方法以取得进一步的发展。

Abdominal massage is an important part of Chinese Tuina medicine,and its clinical efficacy is remarkable,and can be widely used in clinical treatment in various disciplines.With the continuous improvement of research techniques,a more obvious breakthrough has been made in recent years in the study of the mechanism of abdominal massage.This paper summarizes the neurobiological mechanisms associated with abdominal massage treatment in the last two decades,and summarizes that this therapy mainly focuses on digestive,neurological and endocrine system diseases.Abdominal massage treatment can modulate neurotransmitter levels(including monoamine neurotransmitters,peptide neurotransmitters),neuroinflammatory responses and stress responses(including hypothalamic-pituitary-adrenal axis,oxidative stress),neurotrophic factors and gene protein expression(including brain-derived neurotrophic factors,glial cell-derived neurotrophic factors,and other neurological gene protein expression),neuronal activity and metabolites to play a therapeutic role,reflecting the multi-target and bi-directional action of abdominal massage.The paper also discusses the problems of the neurobiological mechanism of abdominal massage and suggests that the research in this field is still in its infancy,mainly based on experimental studies,which has some limitations and still needs to be further developed with the help of systematic scientific research methods.
