中、西医治疗视网膜静脉阻塞的研究进展

Research Progress in the Treatment of Retinal Vein Occlusion with Traditional Chinese Medicine and Western Medicine

视网膜静脉阻塞(RVO)分为视网膜中央静脉阻塞和视网膜分支静脉阻塞,是第二大致盲性眼底血管疾病,仅次于糖尿病视网膜病变,其眼底表现为视网膜静脉迂曲扩张,伴出血、渗出及水肿等,黄斑水肿(ME)是RVO最常见的并发症,反复的ME可导致视网膜光感受器不可逆的损害,是视力下降的重要原因。目前,西医治疗以针对并发症减轻ME为主,中医以活血化瘀为法,可改善全身证候,促进眼底出血吸收、减轻ME。本文从西医的激光治疗、药物治疗、手术治疗,中医的辨证治疗、分期治疗、中成药治疗及中西医结合治疗方面对RVO治疗的相关研究进行综述,以期为临床医生选择最佳的RVO治疗方式提供参考。

Retinal vein occlusion(RVO)is divided into central retinal vein occlusion and branch retinal vein occlusion.It is the second most common fundus vascular disease that could lead to blindness,second only to diabetic retinopathy.Its fundus manifestations include tortuous expansion of retinal veins,accompanied by hemorrhage,exudation,and edema.Macular edema(ME)is the most common complication of RVO.Repeated ME can lead to irreversible damage to retinal photoreceptors,which is an important cause of vision loss.The current treatment of RVO with Western Medicine(WM)mainly focuses on its complications to reduce ME,while the treatment with Traditional Chinese Medicine(TCM)focuses on promoting blood circulation and removing blood stasis,which can improve systemic syndromes,promote the absorption of fundus hemorrhage,and alleviate ME.This paper summarizes the relevant studies on RVO treatment from the aspects of laser therapy,drug therapy and surgical therapy of WM,as well as syndrome differentiation therapy,staging therapy,Chinese patent medicine therapy,and also integrated TCM and WM therapy,in order to provide reference for clinicians to choose the best treatment method for RVO.
