踝三针为主治疗腰椎间盘突出症根性痛的Meta分析

Ankle three-needle as primary treatment for radicular pain in lumbar disc herniation:a Meta-analysis

目的系统评价踝三针为主治疗腰椎间盘突出症根性痛的有效性及安全性。方法计算机检索知网、万方、维普、SinoMed、PubMed、Cochrane Library等数据库,搜集踝三针为主治疗腰椎间盘突出症根性痛的相关文献,检索时限均为建库至2020年12月11日,进行资料提取和质量评价后,使用RevMan 5.3软件进行统计分析。结果①踝三针为主治疗腰椎间盘突出症根性痛的有效率显著高于非踝三针治疗(OR=5.10,95%CI=3.42~7.59,Z=8.00,P<0.05);②踝三针为主治疗腰椎间盘突出症根性痛的VAS评分显著低于非踝三针治疗(WMD=1.26,95%CI=-1.76~-0.77,Z=4.98,P<0.05);③踝三针为主治疗腰椎间盘突出症根性痛的JOA评分显著高于非踝三针治疗(WMD=6.49,95%CI=6.07~6.92,Z=29.79,P<0.05);④踝三针为主治疗腰椎间盘突出症根性痛的镇痛起效时间显著低于非踝三针治疗(WMD=-6.0,95%CI=-6.5~-5.5,Z=23.51,P<0.05);⑤踝三针为主治疗腰椎间盘突出症根性痛的镇痛维持时间显著高于非踝三针治疗(WMD=12.11,95%CI=4.33~19.88,Z=3.05,P<0.05)。结论踝三针为主治疗腰椎间盘突出症根性痛镇痛起效快、维持时间长,可显著提高临床有效率、减轻疼痛水平及改善腰椎活动度,但尚需更多高质量、大样本的随机对照试验进一步验证其疗效。

Objective To systematically evaluate the efficacy and safety of ankle three-needle in the treatment ofroot pain in lumbar disc herniation.Methods The clinical trials regarding ankle three-needle for root pain of lumbar disc herniation published before December 11st 2020 were searched in databases of CNKI,WF,VIP,SinoMed,PubMed,Cochrane Library.The information of included studies was extracted and the quality was assessed by two independent researchers.The Meta analysis was performed by using RevMan 5.3 software.Results(1)The effective rate of ankle three-needle for the root pain of lumbar disc herniation was significantly higher than that of non-ankle three-needle(OR=5.10,95%CI=3.42~7.59,Z=8.00,P<0.05).(2)The VAS score of the ankle threeneedle for root pain of lumbar disc herniation was significantly lower than that of non-ankle three-needle(WMD=1.26,95%CI=-1.76~-0.77,Z=4.98,P<0.05).(3)The JOA score of ankle three-needle for root pain of lumbar disc herniation was significantly higher than that of non-ankle three-needle(WMD=6.49,95%CI=6.07~6.92,Z=29.79,P<0.05).(4)The analgesia onset time of ankle three-needle for root pain of lumbar disc herniation was significantly lower than that of non-ankle three-needle(WMD=-6.0,95%CI=-6.5~-5.5,Z=23.51,P<0.05).(5)The analgesic duration of ankle three-needle for root pain of lumbar disc herniation was significantly higher than that of non-ankle three-needle(WMD=12.11,95%CI=4.33~19.88,Z=3.05,P<0.05).Conclusion Ankle three-needle has a rapid onset and long analgesic duration,which can significantly improve the clinical effectiveness rate,reduce the pain level and improve lumbar functional mobility as primary treatment for lumbar disc herniated radicular pain,but more highquality and large sample randomized controlled trials are needed to further verify its efficacy.
