振幅整合脑电图监测对早产儿神经行为发育状况的评估价值

摘要 目的探究振幅整合脑电图监测对早产儿神经行为发育状况的评估价值。方法选取2020年6月至2022年11月于淄博市妇幼保健院分娩的200例早产儿为研究对象,所有早产儿均行振幅整合脑电图监测,并以随访1个月后的新生儿行为神经测定(NBNA)评分为金标准,按照NBNA评分分为正常组和异常组,比较两组的振幅电压水平、连续原始脑电图比例、显著睡眠觉醒周期比例,分析振幅整合脑电图监测对早产儿神经行为发育的评估效能。结果振幅整合脑电图监测评估早产儿神经行为发育的准确率为98.50%、敏感度为98.67%、特异度为98.40%。异常组振幅电压的上界和下界、原始脑电图连续比例、显著睡眠觉醒周期比例均低于正常组(P<0.05)。结论振幅整合脑电图监测对早产儿神经行为发育状况的评估具有显著的价值,能通过振幅电压的水平及原始脑电图、睡眠觉醒周期的情况反馈其神经行为的发育状况,可作为评估临床早产儿神经行为发育状况的重要手段。 Objective To explore the value of amplitude-integrated electroencephalogram(EEG)monitoring in the evaluation of neurobehavioral development status of premature infants.Methods A total of 200 premature infants delivered in Zibo Maternal and Child Health Hospital from June 2020 to November 2022 were included in the study.All the premature infants underwent amplitude-integrated EEG monitoring,and the newborn behavioral neurological assessment(NBNA)score after 1 month of follow-up was used as the gold standard.The premature infants were divided into normal group and abnormal group according to the score of NBNA.The amplitude voltage levels,continuous original EEG proportion,significant sleep-wake cycle proportion between the two groups were compared,and the value of amplitude-integrated EEG monitoring in evaluating the neurobehavioral development status in premature infants was analyzed.Results The accuracy,sensitivity and specificity of amplitude-integrated EEG monitoring were 98.50%,98.67%and 98.40%.The upper and lower bounds of amplitude voltage levels,the proportions of continuous original EEG and significant sleep-wake cycle in the abnormal group were lower than those in the normal group(P<0.05).Conclusion Amplitude-integrated EEG monitoring is of significant value in the evaluation of the neurobehavioral development status in premature infants by feeding back the amplitude voltage level,the original EEG and the sleepwake cycle,which can be used as the important method to assess the neurobehavioral development status in premature infants.
