超声乳化联合房角分离术治疗急性闭角型青光眼合并白内障的临床研究

Clinical Study of Phacoemulsification Combined with Goniosynechialysis in Treatment of Acute Angle-Closure Glaucoma Complicated with Cataracts

目的:分析超声乳化联合房角分离术治疗急性闭角型青光眼合并白内障的临床效果。方法:选取2020年1月—2023年3月北京市中关村医院收治的44例急性闭角型青光眼合并白内障患者作为研究对象,根据随机数字表法分为对照组与试验组,各22例。对照组采用超声乳化联合人工晶体植入术进行治疗,试验组采用超声乳化联合房角分离术、人工晶体植入术进行治疗。比较两组临床疗效、眼内压力及视力、术后并发症发生情况。结果:试验组治疗总有效率高于对照组,差异有统计学意义(P=0.003)。治疗前,两组视力、眼内压力比较,差异无统计学意义(P>0.05);治疗后,试验组视力高于对照组,眼内压力低于对照组,差异有统计学意义(P<0.001)。试验组并发症发生率低于对照组,差异有统计学意义(P<0.001)。结论:急性闭角型青光眼合并白内障患者实施超声乳化联合房角分离术的治疗效果显著,能够提高视力、降低眼内压力,减少并发症发生。

Objective:To analyze the clinical effect of phacoemulsification combined with goniosynechialysis in the treatment of acute angle-closure glaucoma complicated with cataracts.Methods:A total of 44 patients with acute angle-closure glaucoma complicated with cataracts admitted to Beijing Zhongguancun Hospital from January 2020 to March 2023 were selected as the study subjects,and they were divided into control group and experimental group according to the random number table method,with 22 cases in each group.The control group was treated by phacoemulsification and intraocular lens implantation,and the experimental group was treated by phacoemulsification combined with goniosynechialysis and intraocular lens implantation.The clinical efficacy,intraocular pressure,visual acuity,and postoperative complications of the two groups were compared.Results:The total effective rate of treatment in the experimental group was higher than that in the control group,and the difference was statistically significant(P=0.003).Before treatment,there was no significant difference in visual acuity and intraocular pressure between the two groups(P>0.05).After treatment,the visual acuity in the experimental group was higher than that in the control group,and the intraocular pressure in the experimental group was lower than that in the control group,and the difference was statistically significant(P<0.001).The complication rate in the experimental group was lower than that in the control group,and the difference was statistically significant(P<0.001).Conclusion:For patients with acute angle-closure glaucoma complicated with cataracts,phacoemulsification combined with goniosynechialysis has a significant treatment effect,which can improve visual acuity,reduce intraocular pressure and reduce complications.
