摘要
目的 探讨5-氨基酮戊酸光动力疗法(ALA-PDT)联合手术治疗皮肤基底细胞癌的疗效及安全性。方法根据治疗方法的不同将80例皮肤基底细胞癌分为手术组(n=35)与联合组(n=45),手术组患者给予单纯手术治疗,联合组患者给予手术联合ALA-PDT治疗。手术组患者采用单纯手术治疗,联合组患者在手术组的基础上联合ALA-PDT进行治疗。比较两组患者临床疗效、愈合情况、美容效果、并发症发生情况和复发情况。结果联合组患者的总有效率为97.78%,高于手术组患者的80.00%,差异有统计学意义(P﹤0.05)。治疗后,联合组患者的Ⅰ期愈合率、美容优良率均明显高于手术组,差异均有统计学意义(P﹤0.01)。联合组患者的并发症总发生率和复发率均低于手术组,差异均有统计学意义(P﹤0.05)。结论 ALA-PDT联合手术治疗皮肤基底细胞癌的疗效确切,Ⅰ期愈合率较高,美容效果较好,同时可降低并发症发生率和复发率,安全可靠。
Objective To investigate the efficacy and safety of 5-aminolaevulinic acid photodynamic therapy(ALAPDT)combined with surgery for skin basal-cell carcinoma.Method According to the different treatment methods,80 cases of skin basal-cell carcinoma were divided into the operation group(n=35)and the combination group(n=45).The patients in the operation group were treated with surgery alone,and the patients in the combination group were treated with surgery combined with ALA-PDT.Compared the clinical efficacy,healing status,cosmetic effect,complications,and recurrence of two groups.Result The total effective rate of patients in the combined group was 97.78%,higher than 80.00%in the operation group,the difference was statistically significant(P<0.05).After treatment,the I stage healing rate and cosmetic excellence rate of patients in the combined group were significantly higher than those in the operation group,the differences were statistically significant(P<0.01).The total incidence and recurrence rate of complications in the combined group were lower than those in the operation group,the differences were statistically significant(P<0.05).Conclusion ALA-PDT combined with surgery for skin basal-cell carcinoma has a definite curative effect.It has a I stage healing rate,cosmetic excellence rate,and can reduce the incidence of complications and recurrence rate.It is safe and reliable.
