术前3.0T MRI检查对直肠癌TN分期、环周切缘的诊断效能分析

Diagnostic efficacy of preoperative 3.0T MRI in TN staging and circumferential resection margin of rectal cancer

目的 探讨术前3.0T MRI检查对直肠癌TN分期、环周切缘的诊断效能。方法 选取52例直肠癌患者,均进行术前3.0T MRI检查和术后病理检查。分析术前3.0T MRI检查对直肠癌TN分期以及环周切缘的诊断效能。结果 术前3.0T MRI检查诊断N_(0)期的准确度、灵敏度、特异度分别为92.31%(48/52)、90.91%(20/22)、93.33%(28/30),诊断N_(1)期的准确度、灵敏度、特异度分别为86.54%(45/52)、80.00%(16/20)、90.62%(29/32),诊断N_(2)期的准确度、灵敏度、特异度分别为94.23%(49/52)、90.00%(9/10)、95.24%(40/42)。术前3.0T MRI检查诊断T_(1~2)期的准确度、灵敏度、特异度分别为92.31%(48/52)、85.71%(12/14)、94.74%(36/38),诊断T_(3)期的准确度、灵敏度、特异度分别为84.62%(44/52)、93.94%(31/33)、68.42%(13/19),诊断T_(4)期的准确度、灵敏度、特异度分别为92.31%(48/52)、20.00%(1/5)、100%(47/47)。术前3.0T MRI检查诊断环周切缘阳性的准确度、灵敏度、特异度分别为90.38%(47/52)、88.24%(15/17)、91.43%(32/35)。结论 直肠癌患者术前采用3.0T MRI检查对其TN分期以及环周切缘具有较高的诊断价值,临床上可将术前3.0T MRI检查作为直肠癌患者TN分期以及环周切缘的主要检查手段。

Objective To explore the diagnostic efficacy of preoperative 3.0T MRI in TN staging and circumferential resection margin of rectal cancer.Method A total of 52 patients with rectal cancer were selected,and all patients underwent 3.0T MRI examination and postoperative pathological examination to analyze the diagnostic efficacy of preoperative 3.0T MRI in TN staging and circumferential resection margin of rectal cancer.Result The accuracy,sensitivity and specificity of preoperative 3.0T MRI in the diagnosis of stage N_(0) were 92.31%(48/52),90.91%(20/22)and 93.33%(28/30),respectively;stage N_(1) were 86.54%(45/52),80.00%(16/20)and 90.62%(29/32),respectively;stage N_(2) were 94.23%(49/52),90.00%(9/10)and 95.24%(40/42),respectively.The accuracy,sensitivity and specificity of preoperative 3.0T MRI in the diagnosis of stage T_(1-2) were 92.31%(48/52),85.71%(12/14)and 94.74%(36/38),respectively;stage T_(3) were 84.62%(44/52),93.94%(31/33)and 68.42%(13/19),respectively;stage T_(4) were 92.31%(48/52),20.00%(1/5)and 100%(47/47),respectively.The accuracy,sensitivity and specificity of preoperative 3.0T MRI for positive circumferential resection margin were 90.38%(47/52),88.24%(15/17)and 91.43%(32/35),respectively.Conclusion Preoperative 3.0T MRI for rectal cancer has high diagnostic value,which can be used as the main examination method for preoperative TN staging and circumferential resection margin of patients with rectal cancer.
