SBAR交接班管理模式在神经外科ICU的应用

Application of SBAR shift management mode in neurosurgery ICU

目的探讨现状-背景-评估-建议(situation-back ground-assessment-recommendation,SBAR)模式交接班管理策略对神经外科ICU护士病情理解以及应对能力的影响。方法选取2021年1月—12月1000名患者以及30名护士纳入研究,其中2021年1—6月期间收治的500名患者作为对照组,实施常规床旁交接班模式进行患者交接班工作;2021年7—12月收治的500名患者作为观察组,采用SBAR模式进行床旁交接班。30名护士中男1例,女29例;年龄25~39岁,平均年龄32.19±3.29岁。观察两组患者采用不同交接班模式后其交接班执行率情况、患者对于疾病知晓率、护士对患者病情理解程度与应对能力情况。结果干预后护士对于患者病情理解程度评分与应对能力评分高于干预前,差异有统计学意义(P<0.05)。观察组患者现状情况、交接班背景、交接班评估、交接班建议均优于对照组,差异有统计学意义(P<0.05)。观察组患者病情知晓情况得分高于对照组,差异有统计学意义(P<0.05)。观察组患者整体满意程度高于对照组患者,差异有统计学意义(P<0.05)。结论对于神经外科交接班工作采用SBAR模式进行沟通,条理清晰地帮助交接班医护人员掌握患者病情以及相关治疗重点的同时,也能帮助患者缓解负性情绪,更好的配合医护人员工作。

Objective To explore the infl uence of the situation-back ground-assessment-recommendation(SBAR)model shift management strategy on the condition understanding and coping ability of Neurosurgery ICU nurses.Methods A total of 1000 patients and 30 nurses from January to December 2021 were selected for the study,with 500 patients admitted from January to June 2021 as the control group.The routine bedside shift handover mode was implemented for patient handover work;another 500 patients admitted from July to December 2021 served as the observation group and underwent bedside shift handover using the SBAR mode.Among the 30 nurses,there was 1 male and 29 female;the age was 25-39 years old,with an average age of 32.19±3.29 years.We observed the implementation rate of shift handover,patient awareness of the disease,nurses'understanding of the patient's condition,and coping ability of two groups of patients who adopted different shift handover modes.Results After the intervention,the nurses'understanding and coping ability scores of the patient's conditions were higher than those of the nurses before the intervention,and the diff erences were statistically signifi cant(P<0.05).The observation group had better patient status,handover background,handover evaluation,and handover suggestions than the control group,and the diff erences were statistically signifi cant(P<0.05).The observation group had a higher score of disease awareness than the control group,and the diff erence was statistically signifi cant(P<0.05).The overall satisfaction of the observation group patients was higher than that of the control group patients,and the diff erence was statistically signifi cant(P<0.05).Conclusion The SBAR mode was used for communication in neurosurgery shift work,which can help the shift medical staff grasp the patient's condition and related treatment priorities in a clear way,and can also help patients alleviate negative emotions and better cooperate with medical staff.
