参麦注射液联合美罗培南治疗重症肺炎的临床研究

Clinical Study on the Treatment of Severe Pneumonia by Shenmai Injection Combined with Meropenem

目的 探讨参麦注射液联合美罗培南在重症肺炎治疗中的应用价值。方法 方便选取2020年6月-2022年10月福州市第一医院收治的68例重症肺炎患者为研究对象,按照不同治疗方案分为对照组和研究组,每组34例。对照组予以美罗培南治疗,研究组予以参麦注射液联合美罗培南治疗。比较两组治疗前后动脉血气指标、细胞免疫功能指标、肺功能指标及血清炎症因子水平,记录两组临床症状改善用时。结果 治疗后,研究组氧合指数、血氧分压、CD4^(+)/CD8^(+)、CD4^(+)及肺功能指标高于对照组,二氧化碳分压、CD8^(+)及血清炎症因子均低于对照组,差异有统计学意义(P<0.05)。研究组肺部炎症吸收1/2以上用时(6.17±1.29)d、痰液颜色改变用时(4.53±0.86)d、退热用时(2.69±0.54)d、咳嗽消失用时(7.08±1.46)d均短于对照组的(7.82±1.43)、(5.92±1.20)、(3.78±0.82)、(8.35±1.71)d,差异有统计学意义(t=4.996、5.490、6.473、3.294,P<0.05)。结论 参麦注射液与美罗培南联合应用于重症肺炎的效果较好,可提升患者细胞免疫功能,减轻机体炎症反应,改善肺功能和动脉血气指标,缩短患者临床症状改善时间。

Objective To investigate the application value of Shenmai injection combined with meropenem in the treatment of severe pneumonia.Methods 68 patients with severe pneumonia admitted to Fuzhou First Hospital from June 2020 to October 2022 were conveniently selected as the research objects and divided into control group and study group according to different treatment plans,with 34 cases in each group.The control group was treated with meropenem,and the study group was treated with Shenmai injection combined with meropenem.Arterial blood gas indexes,cellular immune function indexes,lung function indexes and serum inflammatory factors were compared between the two groups before and after treatment,and the improvement time of clinical symptoms in the two groups was recorded.Results After treatment,the oxygenation index,arterial oxygen partial pressure,CD4^(+)/CD8^(+),CD4^(+) and pulmonary function indexes of the study group were higher than those of the control group,arterial partial pressure of carbon dioxide,CD8^(+) and serum inflammatory factors were lower than those of control group,and the difference was statistically significant(P<0.05).The time of lung inflammation absorption by more than half(6.17±1.29)d,sputum color change(4.53±0.86)d,fever reduction(2.69±0.54)d and cough disappearance(7.08±1.46)d in the study group were shorter than thoses in the control group(7.82±1.43)d,(5.92±1.20)d,(3.78±0.82)d,(8.35±1.71)d,the difference was statistically significant(t=4.996,5.490,6.473,3.294,P<0.05).Conclusion Shenmai injection combined with meropenem has a good effect on severe pneumonia,which can improve the cellular immune function of patients,reduce the inflammatory response of the body,improve lung function and arterial blood gas indexes,and shorten the improvement time of clinical symptoms of patients.
