骨折一号汤联合微创手术对肱骨干中段骨折患者术后结局的影响

Effect of Fracture No.1 Decoction Combined with Minimally Invasive Surgery on Postoperative Outcomes of Patients with Middle Humerus Trunk Fracture

目的 探讨对肱骨干中段骨折在微创手术同时联用骨折一号汤干预对改善肩关节功能、骨质量指标及代谢指标的作用。方法 简单随机选取2021年1-12月莱阳市中医医院收治的肱骨干中段骨折者80例为研究对象,以随机数表法分为微创手术治疗对照组与联合骨折一号汤干预观察组,每组40例。随访6月,比较两组关节功能,于术前、术后6周检测骨质量指标以及骨代谢指标进行分析,并比较并发症发生率。结果观察组肩关节功能优良率较对照组高(95.00%vs 80.00%),差异有统计学意义(χ^(2)=4.114,P<0.05)。术后观察组骨体积分数、体积有机质含量、骨小梁数量均大于对照组,骨弹性应力小于对照组,差异有统计学意义(t=7.772、6.102、15.720、21.194,P<0.05)。术后观察组碱性磷酸酶、骨钙素大于对照组,Ⅰ型胶原羧基端肽β小于对照组,差异有统计学意义(t=3.087、4.855、5.926,P<0.05)。观察组并发症率低于对照组(5.00%vs20.00%),差异有统计学意义(χ^(2)=4.114,P<0.05)。结论 对肱骨干中段骨折患者在微创手术治疗基础上联用骨折一号汤干预能提高肩关节功能,有利于提高骨质量及骨代谢状态,能降低并发症发生率。

Objective To investigate the effects of minimally invasive surgery combined with Fracture No.1 decoction on the improvement of shoulder joint function,bone quality indicators and metabolic indicators in patients with middle humerus fracture.Methods A total of 80 patients with midsection humeral shaft fracture admitted to Laiyang Hospital of Traditional Chinese Medicine from January to December 2021 were randomly selected as the study objects,and were divided into the minimally invasive surgery treatment control group and the combined fracture-No.1 decoction intervention observation group,according to the random number table,with 40 cases in each group.Follow-up for 6 months,the joint function of the two groups was compared,bone quality indexes and bone metabolism indexes were detected before and 6 weeks after surgery for analysis,and the incidence of complications was compared.Results The excellent and good rate of shoulder joint function in observation group was higher than that in control group(95.00%vs 80.00%),and the difference was statistically significant(χ^(2)=4.114,P<0.05).After operation,bone volume fraction,volume organic matter content and trabecular number in the observation group were higher than those in the control group,and ES was lower than those in the control group,and the difference was statistically significant(t=7.772,6.102,15.720,21.194,P<0.05).Bone alkaline phosphatase and osteocalcin in the observation group were higher than those in the control group,and type I collagen carboxy-terminal peptideβwas lower than those in the control group,and the difference was statistically significant(t=3.087,4.855,5.926,P<0.05).The complication rate of observation group was lower than that of control group(5.00%vs 20.00%),and the difference was statistically significant(χ^(2)=4.114,P<0.05).Conclusion On the basis of minimally invasive surgical treatment,the combination of fracture-1 decoction can improve the function of shoulder joint,improve bone quality and metabolic status,and reduce the incidence of complications.
