摘要
目的:统计2017-2018年武汉地区女性人乳头瘤病毒(HPV)感染状况,并分析HPV疫苗相关型别(6、11、16、18、31、33、45、52、58)的年度变化及年龄分布特征。方法:选取2017-2018年在我院进行HPV分型检测的10760例成年女性,采用流式荧光杂交法检测27型HPV感染情况。将受检者按18-30岁、31-44岁、45-54岁、55-64岁、≥65岁进行年龄分组;将HPV型别按是否为疫苗涵盖的九类亚型将其分为疫苗相关型别组和非疫苗相关型别组。统计分析不同年度、不同年龄层HPV检出率。结果:纳入的10760例受检者中,27型HPV阳性检出率为19.80%。疫苗相关型别HPV阳性检出率为11.05%,其中高危型和低危型检出率分别为9.42%、3.41%。年龄分层分析显示,疫苗相关型别组HPV检出率随年龄增加而降低,其中18-30岁组检出率显著高于其它年龄组(P<0.05)。疫苗相关型别组中多重HPV亚型感染者共152例,混合感染率为1.41%,主要分布在低龄组。检出率较高的HPV亚型依次为:52、58、16、6型。结论:2017-2018年,武汉地区女性HPV整体感染率较高,以HPV疫苗相关型别感染为主。
Objective:To investigate the prevalence of human papillomavirus(HPV)infection among women in Wuhan from 2017 to 2018,and to analyze the annual variation and age distribution of HPV vaccine-related types(6,11,16,18,31,33,45,52,58).Method:10760 female patients aged≥18 years who underwent HPV typing testing from 2017 to 2018 were selected,and the 27 types of HPV infection status were detected using flow cytometry fluorescence hybridization.Subjects were divided into age groups of 18-30 years old,31-44 years old,45-54 years old,55-64 years old,and≥65 years old.HPV subtypes were divided into vaccine-related and non-vaccine-related subtype groups based on whether they were nine subtypes covered by the vaccine.HPV detection rates in different years and age groups were analyzed.Results:Among the 10760 subjects included,the positive detection rate of 27 types of HPV was 19.80%.The positive detection rate of vaccine related HPV types was 11.05%,with the detection rates of high-risk and low-risk types were 9.42%and 3.41%,respectively.Age stratified analysis showed that the detection rate of HPV in vaccine related subtypes was decreased with age,with a significantly higher detection rate in the 18-30 years old group compared to other age groups(P<0.05).There were 152 cases of mixed infection with multiple HPV subtypes in the vaccine-related type group,and the mixed infection rate of the population was 1.41%,mainly distributed in the younger group.The HPV subtypes with higher detection rates were type 52,58,16 and 6.Conclusion:From 2017 to 2018,the overall infection rate of HPV among women in Wuhan was relatively high,with HPV vaccine related types being the main type of infection.
