男性高尿酸血症合并2型糖尿病患者血清FGF21水平变化及意义探讨

Changes and Significance of Serum FGF21 Levels in Male Patients with Hyperuricemia and Type 2 Diabetes Mellitus

目的:探讨男性高尿酸血症(HUA)合并2型糖尿病(T2DM)患者血清成纤维细胞生长因子21(FGF21)水平变化及临床意义。方法:纳入2022-05-07-2022-12-11于本院内分泌科就诊的175例HUA患者,根据是否合并T2DM,将其分为HUA-T2DM(n=79)和HUA组(n=96),纳入同期就诊的88例UA正常男性作为对照组(n=88)。采用ADIVA-2400全自动生化分析仪检测空腹血清尿酸(UA)、糖化血红蛋白(HbA1c)、血糖(FPG)、尿素(Urea)、肌酐(Cr)和补体C1q水平,计算预估肾小球滤过率(eGFR);采用酶免疫技术定量检测血清FGF21水平,比较三组间上述指标的差异。采用Logistic回归分析评估HUA患者血清FGF21与T2DM风险之间的关系。结果:与对照组相比,HUA组UA、C1q和FGF21水平升高,HUA-T2DM组FPG、HbA1c、UA、Cr、Urea、C1q和FGF21升高,eGFR降低;与HUA组相比,HUA-T2DM组FPG、HbA1c、UA、Cr、Urea、C1q和FGF21均升高,差异均具有统计学意义(P<0.05)。Logistic回归分析显示,充分调整协变量后,FGF21均与男性HUA患者发生T2DM呈正相关[OR及95%CI:1.240(1.090-1.290),P<0.05]。结论:男性HUA患者血清FGF21水平升高,其水平升高与男性HUA患者发生T2DM有关。

Objective:To investigate the changes and significance of serum fibroblast growth factor 21(FGF21)in male patients with hyperuricemia(HUA)and type 2 diabetes mellitus(T2DM).Method:From May 2022 to December 2022,175 male patients with HUA were enrolled.According to the diagnostic criteria of T2DM,they were divided into HUA group(n=96)and HUA-T2DM group(n=79).During the same period,88 men with normal UA concentrations were enrolled as the control group(n=88).The uric acid(UA),glycosylated hemoglobin(HbA1c),fasting plasma glucose(FPG),Urea,creatinine(Cr)and complement C1q levels were detected in ADIVA-2400 automatic biochemical analyzer,and the estimated glomerular filtration rate(eGFR)was then calculated.Serum FGF21 levels were quantified by enzyme immunoassay,and the indicator differences among the three groups were compared.Logistic regression analysis was conducted to evaluate the relationship between FGF21 and the risk of T2DM in HUA men.Results:Compared with the control group,the levels of UA,C1q and FGF21 were higher in the HUA group,and the levels of FPG,HbA1c,UA,Cr,Urea,C1q and FGF21 were increased in the HUA-T2DM group.However,the eGFR was decreased.In addition,compared with the HUA group,FPG,HbA1c,UA,Cr,Urea,C1q and FGF21 in the HUA-T2DM group were increased,and all differences were statis-tically significant(P all<0.05).After fully adjusting covariates,Logistic regression analysis showed that FGF21 was positively correlated with T2DM in HUA men[OR and 95%CI:1.240(1.090-1.290),P<0.05].Conclusion:FGF21 is increased in male patients with HUA,and its elevation has a reference value for evaluating the occurrence of T2DM in male patients with HUA.
