血氧指数、血脂在主动脉夹层伴阻塞性睡眠呼吸暂停低通气综合征患者中的表达水平

Levels of Blood Oxygen Index and Blood Lipids in Aortic Dissection Patients with Obstructive Sleep Apnea Syndrome

目的研究主动脉夹层(AD)伴阻塞性睡眠呼吸暂停低通气综合征(OSAHS)患者的血氧指数、血脂水平。方法本研究招募了来自河南省胸科医院的40例急性AD患者(AD组)和40例非AD患者(对照组);所有患者接受STOP-BANG问卷评分及夜间多导睡眠监测。STOP-BANG问卷≥5分的AD患者定义为OSAHS组,STOP-BANG问卷<5分的AD患者定义为非OSAHS组,观察AD组睡眠呼吸暂停低通气指数(AHI)、≥4%氧减指数、血氧饱和度、最低血氧饱和度指数等。采集所有患者静脉血,检测患者甘油三酯、低密度脂蛋白、高密度脂蛋白、超敏C反应蛋白、血清同型半胱氨酸(Hcy)、血清胱抑素C(CysC)等指标。结果AD组OSAHS患者占比高于对照组(χ^(2)=4.713,P=0.030)。OSAHS组和非OSAHS组性别、年龄、血氧饱和度、最低血氧饱和度、高密度脂蛋白、超敏C反应蛋白比较差异无统计学意义(P>0.05);OSAHS组体重指数、STOP-BANG评分、AHI、≥4%氧减指数、甘油三酯、低密度脂蛋白、Hcy高于非OSAHS组,OSAHS组CysC低于非OSAHS组(P<0.05)。结论AD伴OSAHS患者肺通气功能低于AD不伴OSAHS患者,低密度脂蛋白、Hcy高于AD不伴OSAHS患者,血氧指数、血脂对AD患者是否伴有OSAHS具有一定的指导意义。

Objective To study the level of blood oxygen and lipid index in patients with aortic dissection(AD)with obstructive sleep apnea syndrome(OSAHS).Methods A total of 40 acute AD patients(AD group)and 40 non-AD patients(control group)from Henan Provincial Chest Hospital were recruited,all patients received STOP-BANG questionnaire scores and nocturnal polysomnography monitoring.AD patients with a score of≥5 on the STOP-BANG questionnaire were defined as the OSAHS group,while AD patients with a score of<5 on the STOP-BANG questionnaire were defined as the non-OSAHS group.The sleep apnea-hypopnea index(AHI),≥4%oxygen desaturation index,average oxygen saturation,minimum oxygen saturation index of all patients in the AD group were observed.The venous blood of all patients was collected,and the indexes of triglyceride,low-density lipoprotein,high-density lipoprotein,high-sensitivity C-reactive protein,serum homocysteine(Hcy)and serum Cystatin C(CysC)were detected.Results OSAHS patients in the AD group were more than those in the control group(χ^(2)=4.713,P=0.030).There was no statistical difference in gender,age,mean oxygen saturation,lowest oxygen saturation,high-density lipoprotein and high-sensitivity C-reactive protein between the OSAHS group and the non-OSAHS group(P>0.05).The level of body mass index,STOP-BANG score,AHI,≥4%oxygen reduction index,triglyceride,low-density lipoprotein and Hcy in the OSAHS group were higher than those in the non-OSAHS group,and the level of CysC in the OSAHS group was lower than that in the non-OSAHS group(P<0.05).Conclusion The lung ventilation function of AD patients with OSAHS is lower than that of AD patients without OSAHS,while low-density lipoprotein and Hcy are higher than those of AD patients without OSAHS.Blood oxygen index and blood lipids have certain guiding significance for AD patients whether they have OSAHS.
