摘要
目的观察急性心力衰竭患者极早期使用无创呼吸机辅助通气的临床疗效。方法选取2022年1~12月于本院急诊科就诊的急性心力衰竭患者66例作为研究对象,随机将患者分为对照组与实验组,每组33例。对照组采用常规治疗方法,患者症状不能缓解的情况下开始使用无创呼吸机辅助通气。实验组在对照组常规治疗的基础上,于急性心力衰竭发生的极早期(30 min内)进行无创呼吸机辅助通气治疗。对比两组患者治疗后的血压、血氧饱和度、心率及呼吸困难缓解时间,以及氧分压(PaO2)、二氧化碳分压(PaCO_(2))、乳酸(Lac)水平。结果治疗后,实验组PaO2高于对照组,差异有统计学意义(P<0.05)。治疗前后,实验组收缩压、血氧饱和度、心率的差值大于对照组;治疗后,实验组呼吸困难缓解时间小于对照组,差异有统计学意义(P<0.05)。结论极早期使用无创呼吸机辅助通气治疗急性心力衰竭患者有显著疗效。
Objective To observe the clinical efficacy of extremely early use of non-invasive ventilator(NIV)assisted ventilation in patients with acute heart failure.Methods A total of 66 patients with acute heart failure treated in the emergency department of our hospital from January to December 2022 were selected as research objects.The patients were randomly divided into control group and experimental group,with 33 cases in each group.The control group received routine treatment,and NIV assisted ventilation was initiated only if the patients'symptoms were not relieved.The experimental group received NIV assisted ventilation in the extremely early stage of acute heart failure(within 30 minutes)on the basis of routine treatment in the control group.The Blood pressure,oxygen saturation,heart rate,remission time of dyspnea,partial pressure of oxygen(PaO2),partial pressure of carbon dioxide(PaCO_(2))and lactic acid(Lac)before and after treatment were compared between the two groups.Results After treatment,PaO2 of the experimental group was higher than that of the control group,the difference was statistically significant(P<0.05).Before and after treatment,the difference of systolic pressure,blood oxygen saturation and heart rate in the experimental group was greater than that in the control group.After treatment,the remission time of dyspnea in the experimental group was less than that in the control group,and the difference was statistically significant(P<0.05).Conclusion Extremely early use of NIV assisted ventilation in the treatment of patients with acute heart failure has shown significant therapeutic effects.
