电子结肠镜下冷圈套切除术对较小结直肠扁平无蒂息肉治疗的临床疗效及安全性分析

Clinical Efficacy and Safety Analysis of Electronic Colonoscope Cold Loop Resection for Smaller Colorectal Flat Non-tip Polyp Treatment

目的探讨较小的结直肠扁平无蒂息肉患者接受电子结肠镜下冷圈套切除术治疗的临床疗效及安全性。方法回顾性分析2020年2月—2021年2月滨州医学院附属医院收治的780例较小结直肠扁平无蒂息肉患者的临床资料,根据息肉切除方法不同,分为观察组及对照组。观察组390例患者接受电子结肠镜下冷圈套切除术治疗,对照组390例患者接受电子结肠镜下高频电凝切除术治疗。对比两组结肠息肉切除手术指标、手术并发症发生情况及住院时间情况。结果观察组临床总有效率为97.69%,高于对照组的78.72%,差异有统计学意义(χ^(2)=67.481,P=0.001)。观察组手术时间、住院时间均短于对照组,差异有统计学意义(P<0.05)。观察组手术并发症总发生率为7.95%,明显低于对照组的13.59%,差异有统计学意义(χ^(2)=6.457,P=0.011)。两组的息肉完整切除率值、息肉标本回收率值对比,差异无统计学意义(P>0.05)。结论给予较小结直肠扁平无蒂息肉患者取电子结肠镜下冷圈套切除术治疗,相比于电凝电切手术时间更短,住院时间短,临床疗效显著。

Objective To investigate the clinical efficacy and safety of smaller colorectal flat non-tip polyps in pa⁃tients treated with electronic colonoscope cold loop resection.Methods The clinical data of 780 patients with small colorectal flat sessile polyps admitted to the Affiliated Hospital of Binzhou Medical University from February 2020 to February 2021 were retrospectively analyzed.According to different polyp resection methods,they were divided into observation group and control group.390 patients in the observation group were treated with cold snare resection un⁃der electronic colonoscopy,and 390 patients in the control group were treated with high-frequency electrocoagulation resection under electronic colonoscopy.The surgical indexes,surgical complications and hospitalization time of colon polypectomy in the two groups were compared.Results The total clinical effective rate of the observation group was 97.69%,which was higher than 78.72%of the control group,and the difference was statistically significant(χ^(2)=67.481,P=0.001).The operation time and hospitalization time of the observation group were shorter than those of the control group,the difference was statistically significant(P<0.05).The total incidence of surgical complications of ob⁃servation group was 7.95%,which was significantly lower than 13.59%of the control group,and the difference was statistically significant(χ^(2)=6.457,P=0.011).There was no significant difference in polyp complete resection rate and polyp specimen recovery rate between the two groups(P>0.05).Conclusion Giving smaller colorectal flat non-tip polyp patients to take the electronic colonoscopy cold loop resection treatment,compared with electrocoagulation and electric resection operation time is shorter,hospitalization time is short,the clinical efficacy is remarkable.
