替米沙坦联合用药治疗老年高血压的临床效果分析

Analysis of the Clinical Effect of Telmisartan Combined Therapy in the Treatment of Elderly Hypertension

目的探讨对老年高血压患者给予替米沙坦联合用药治疗后获得临床效果。方法选取2020年3月—2022年6月山东省临沂市中心医院120例老年高血压患者作为研究对象,以投掷硬币法分组,其中施以替米沙坦片用药治疗的设为参照组(n=60);施以替米沙坦片+苯磺酸氨氯地平分散片用药治疗的设为研究组(n=60)。对比两组老年高血压患者治疗总有效率、不良反应总发生率、血压波动情况、白细胞介素-8(interleu⁃kin-8,IL-8)、白细胞介素-6(interleukin-6,IL-6)、肿瘤坏死因子-α(tumor necrosis factor,TNF-α)、细胞间黏附分子-1(intercellular adhesion molecule-1,ICAM-1)、纤维蛋白原(fibrinofen,FIB)、超敏C反应蛋白(hyper⁃sensitive C-reactive protein,hs-CRP)。结果研究组治疗总有效率高于参照组,差异有统计学意义(P<0.05)。研究组不良反应总发生率低于参照组,差异有统计学意义(P<0.05)。治疗前,两组收缩压水平以及舒张压水平比较,差异无统计学意义(P>0.05);治疗后,研究组收缩压水平以及舒张压水平低于参照组,差异有统计学意义(P<0.05)。研究组IL-8、IL-6、TNF-α、ICAM-1水平低于参照组,差异有统计学意义(P<0.05)。研究组FIB(2.76±0.89)g/L以及hs-CRP(1.71±0.75)mg/L水平低于参照组(5.85±0.99)g/L、(4.59±0.92)mg/L,差异有统计学意义(t=17.979、18.794,P<0.05)。结论临床对老年高血压患者采用替米沙坦片+苯磺酸氨氯地平分散片进行治疗,可提升治疗效果显著,减少恶心、头痛以及干咳等不良反应,有效降低舒张压水平、收缩压水平,降低IL-8、IL-6、TNF-α、ICAM-1、FIB以及hs-CRP水平,可促进老年高血压患者良好预后。

Objective To explore the clinical effect of telmisartan combined therapy on elderly hypertensive patients.Methods Selecting 120 elderly hypertensive patients from Linyi Central Hospital in Shandong Province from March 2020 to June 2022 as the research subjects.Grouped by coin tossing method,with those treated with telmisartan tab⁃lets as the reference group(n=60);the study group was treated with telmisartan tablets and amlodipine besylate dis⁃persible tablets(n=60).The total effective rate,total incidence of adverse reactions,blood pressure fluctuations,inter⁃leukin-8(IL-8),interleukin-6(IL-6),and tumor necrosis factor-α(TNF-α),intercellular adhesion molecule-1(ICAM-1),fibrinogen(FIB),and hypersensitive C-reactive protein(hs-CRP)between two groups of elderly hyperten⁃sive patients were compared.Results The total effective rate of the study group was higher than that of the reference group,and the difference was statistically significant(P<0.05).The total incidence of adverse reactions in the study group was lower than that in the reference group,and the difference was statistically significant(P<0.05).Before treatment,there was no statistically significant difference in systolic and diastolic blood pressure levels between the two groups(P>0.05);after treatment,the systolic and diastolic blood pressure levels in the study group were lower than those in the reference group,and the difference was statistically significant(P<0.05).Research group the levels of IL-8,IL-6,TNF-α,ICAM-1 level of the research group were lower than those of the reference group,and the difference was statistically significant(P<0.05).The levels of FIB(2.76±0.89)g/L and hs-CRP(1.71±0.75)mg/L in the study group were lower than those in the reference group(5.85±0.99)g/L and(4.59±0.92)mg/L,and the difference was sta⁃tistically significant(t=17.979,18.794,P<0.05).Conclusion Clinical treatment of elderly hypertensive patients with telmisartan tablets and amlodipine besylate dispersible tablets can significantly improve the treatment effect,reduce adverse reactions such as nausea,headache,and dry cough,effectively reduce diastolic and systolic blood pressure levels,and lower IL-8,IL-6,and TNF-α,ICAM-1,FIB,and hs-CRP can promote a good prognosis in elderly hyper⁃tensive patients.
