老年性聋助听器验配效果影响因素Logistic回归分析

Logistic Regression Analysis of Influencing Factors of Hearing Aid Fitting for Senile Deafness

目的:探讨Logistic回归分析老年性聋患者助听器验配效果及其影响因素。方法:纳入2019年1月~2020年5月天津市残疾人康复服务指导中心耳鼻喉科收治的55例老年性耳聋患者作为研究对象,患者均配戴4通道以上的全数字助听器,均为初次配戴。助听器配戴前进行宣教配戴助听器的相关知识,根据患者纯音测听报告合理选择助听器类型,重点对最大声输出和增益等参数做合理调整。采用助听器效果国际调查问卷(IOI-HA)评价助听器效果及满意度。结果:助听3个月后,55例患者的IOI-HA评分均值为21~33分,总均值为(27.6±3.1)分。Logistic回归显示影响助听器验配效果的影响因素主要有验配意愿、助听后平均听阈、助听前言语识别率,与单/双耳助听无关。结论:老年性聋助听器验配效果与老年患者的助听前言语识别率有关,识别率越高助听器验配效果越好。

Objective:To investigate the effect and influencing factors of hearing aid fitting in senile deafness patients by Logistic regression analysis.Methods:From January 2019 to May 2020,55 patients with senile deafness were enrolled in Department of Otolaryngology,Tianjin Rehabilitation Service and Guidance Center for the Disabled as the study subjects.All patients were wearing full digital hearing AIDS with more than 4 channels for the first time.Before wearing hearing AIDS to teach the relevant knowledge of wearing hearing AIDS,according to the patient pure tone audiometry report to choose the hearing aid type,focus on the loudest output and gain and other parameters to make reasonable adjustment.The national Hearing Aid Efficacy Questionnaire(IOI-HA)was used to evaluate the efficacy and satisfaction of hearing AIDS.Results:After 3 months of hearing aid,the mean IOI-HA score of 55 patients was 21-33 points,the total mean was(27.6±3.1)points.Logsitic regression showed that the main factors affecting the effect of hearing aid were willingness to test,average hearing threshold after hearing aid,and speech recognition rate before hearing aid,which had nothing to do with mono/binaural hearing aid.Conclusion:The effect of hearing aid on senile deafness is related to the rate of speech recognition before hearing,the higher the rate of recognition,the better the effect of hearing aid.
