综合康复结合甲钴胺治疗糖尿病神经源性膀胱患者的临床效果观察

Clinical effect of comprehensive rehabilitation combined with mecobalamin on neurogenic bladder in patients with diabetes

目的探讨综合康复结合甲钴胺在糖尿病神经源性膀胱患者中的临床治疗效果。方法选取2021年1月—2022年9月树兰(杭州)医院收治的糖尿病神经源性膀胱患者,利用随机数字表法将患者分为试验组和对照组。对照组采用甲钴胺治疗,试验组采用综合康复结合甲钴胺治疗。比较两组患者的治疗效果、治疗前后的膀胱功能情况、心理状态[抑郁自评量表(SDS)、焦虑自评表(SAS)]评分、生活质量评分以及治疗满意度。结果最终纳入112例患者,其中试验组和对照组各56例。两组治疗总有效率差异无统计学意义(P>0.05),但试验组的临床疗效优于对照组(P<0.05)。治疗后,两组的SAS评分、SDS评分和膀胱残余尿量均较前下降(P<0.01),两组的平均尿流量、角色功能评分、社会功能评分、躯体功能评分、心理功能评分均较前升高(P<0.01);且试验组上述各项指标均明显优于对照组(P<0.01)。结论综合康复结合甲钴胺治疗糖尿病神经源性膀胱患者具有更好的疗效,能改善膀胱功能、减少负面情绪、提高生活质量。

Objective To explore the clinical therapeutic effect of comprehensive rehabilitation combined with mecobalamin in patients with neurogenic bladder of diabetes.Methods The patients with neurogenic bladder of diabetes who were admitted to Shulan(Hangzhou)Hospital from January 2021 to September 2022 were selected.The patients were divided into experimental group and control group according to random number table.The patients of control group were treated with methylcobalamin,while the patients in the experimental group were treated with comprehensive rehabilitation combined with methylcobalamin.The treatment efficacy,bladder function before and after treatment,psychological status[self rating depression scale(SDS),self rating anxiety scale(SAS)]scores,quality of life scores,and treatment satisfaction between two groups of patients were compared.Results There was no statistically significant difference in the total effective rate between two groups(P>0.05),but the clinical efficacy of the experimental group was better than that of the control group(P<0.05).After treatment,the SAS score,SDS score and residual urine volume of both groups decreased compared to before(P<0.01),while the average urine flow,role function score,social function score,physical function score,and psychological function score of both groups increased compared to before(P<0.01).The above indicators of the experimental group were significantly better than those of the control group(P<0.01).Conclusion Comprehensive rehabilitation combined with mecobalamin has better efficacy in treating neurogenic bladder patients with diabetes,which can improve bladder function,reduce negative emotions,improve quality of life.
