萧山地区消化道肿瘤患者营养风险筛查及影响因素分析

Nutritional risk screening and analysis of influencing factors in patients with digestive tract tumors in Xiaoshan Area

摘要 目的探究萧山地区消化道肿瘤患者发生营养风险的情况,分析其影响因素。方法648例消化道肿瘤患者,采用NRS 2002简表实施营养风险筛查,并分析发生营养风险的影响因素。结果648例消化道肿瘤患者中,271例存在营养不足,占41.82%;有营养风险的患者398例,占61.42%。年龄、性别、病程及文化水平对营养不足的影响差异无统计学意义(均P>0.05),居住地类型、肿瘤TNM分期及是否接受化疗对营养不足的影响差异有统计学意义(均P<0.05)。Logistic回归分析显示,居住地类型为农村,肿瘤TNM分期为Ⅲ、Ⅳ期,有化疗史是患者营养不足的影响因素(均P<0.05);年龄、文化水平、居住地类型、肿瘤TNM分期及是否接受化疗对营养风险的影响差异有统计学意义(均P<0.05);高龄,低文化水平,居住地类型为农村,肿瘤TNM分期为Ⅲ、Ⅳ期,有化疗史是患者发生营养风险的影响因素(均P<0.05)。结论萧山地区消化道肿瘤患者普遍存在营养不足情况,营养风险较高,年龄、文化水平、居住地类型、肿瘤分期以及化疗史均是引起营养风险的因素。 Objective To explore the nutritional risk in patients with gastrointestinal tumors in Xiaoshan and analyze its influencing factors.Methods Nutritional risk screening was conducted on 648 patients with digestive tract tumors using the NRS 2002 table,and the factors influencing nutritional risk were analyzed.Results Among the 648 patients diagnosed with gastrointestinal tumors,271 cases(41.82%)were identified as undernourished while 398 patients(61.42%)exhibited nutritional risk.There were no statistically significant differences observed in the effects of age,gender,disease duration and education level on undernutrition(all P>0.05).However,there were statistical differences noted in the effects of type of residence,tumor TNM staging and receipt of chemotherapy(all P<0.05).Logistic regression analysis revealed that the type of rural residence,tumor TNM stageⅢandⅣ,as well as chemotherapy history were significant influencing factors for undernutrition in patients(all P<0.05).The variables of age,education level,type of residence,tumor TNM staging and chemotherapy treatment were found to have statistically significant impacts on nutritional risk(all P<0.05).Specifically,advanced age,low education level,rural residency status,tumor stagesⅢandⅣas well as a history of chemotherapy were identified as influential factors for nutritional risk in patients(all P<0.05).Conclusions Patients with digestive tract tumors in the Xiaoshan commonly experience nutritional deficiencies and are at high risk for malnutrition.Age,education level,residential status,tumor stage,and chemotherapy history all contribute to this increased risk.
