癌症患者PICC置管支持性照护需求量表的编制及信效度检验

Development of Supportive Care Need Scale for cancer patients with PICC and its reliability and validity

目的 构建PICC置管癌症患者支持性照护需求量表,并进行信效度检验。方法 以支持性照护需求理论框架为基础,通过文献回顾、质性访谈、德尔菲专家咨询法以及预调查形成量表的初稿。选取浙江省3所三级甲等医院置管癌症患者277例进行调查,测定量表的信效度。结果 最终形成的PICC置管癌症患者支持性照护需求量表包括生理需求、心理需求、精神需求、信息需求、社会需求、实际需求6个维度,共29个条目。量表的内容效度为0.931,各条目的内容效度为0.875~1.000。探索性因子分析提取了6个公因子,累积方差贡献率为68.194%;验证性因子分析显示,量表因素结构稳定。总量表的Cronbach α系数为0.926,各维度的Cronbach α系数在0.716~0.903,量表的折半系数为0.778,各维度折半系数在0.716~0.915。量表各维度得分及总分与癌症患者支持性照护需求量表(SCNS-SF34)总分及各维度呈正相关(P<0.001)。结论 形成的PICC置管癌症患者支持性照护需求量表的信效度良好,可用于PICC置管癌症患者支持性照护需求的评估。

Objective To develop a Supportive Care Need Scale for Cancer Patients with PICC,and to test its reliability and validity.Methods Under the theoretical framework of supportive care needs,after literature review and qualitative interview,the draft of the scale was developed by using Delphi method and preliminary survey.A total of 277 cancer patients with PICC discharged from 3 tertiary grade-A hospitals in Zhejiang province were selected as subjects.The reliability and validity of the scale were tested.Results The final scale included the following 6 dimensions:physiological need,psychological need,information need,emotional need,social need,and practical need,involving 29 items.The content validity index was 0.931,and the content validity indexes of each items ranged from 0.875 to 1.000.Exploratory factor analysis(EFA)extracted 6 factors,whose cumulative contribution of variance accounted for 68.194%;confirmatory factor analysis showed that the scale factor structure was stable.The Cronbachαcoefficient of the total scale was 0.926,and the Cronbachαcoefficient of each dimension 0.716~0.903.The split reliability coefficient of the scale was 0.788,and the split reliability coefficient of each dimension was 0.716~0.915.The total score of the developed scale and the score of each dimension were positively correlated with total score of SCNS-SF34 and the score of each dimension(P<0.001).Conclusion Supportive Care Needs Scale for Cancer Patients with PICC has good reliability and validity.The scale can be used as a tool to evaluate supportive care needs of cancer patients with PICC.
