视频脑电图监测对继发性癫痫的诊断价值

Diagnostic value of video electroencephalography monitoring for secondary epilepsy

目的探讨视频脑电图监测对继发性癫痫的诊断价值。方法抽取2019年1月至2022年6月驻马店市中心医院收治的继发性癫痫患者68例,均行视频脑电图及常规脑电图检测。比较两种检查方式对继发性癫痫阳性、痫样放电、异样脑电图信号、同步发作、不同发生部位的检出情况。结果68例患者中视频脑电图对继发性癫痫阳性检出率(94.12%,64/68)高于常规脑电图(82.35%,6/68),P<0.05。视频脑电图对癫样放电、异样脑电图信号及同步发作的检出率(48.53%,33/68;89.71%,61/68;35.29%,24/68)高于常规脑电图(35.29%,24/68;76.47%,52/68;13.24%,9/68),P<0.05。视频脑电图对深部白质区、脑叶皮质区有癫痫样波的检出率(52.94%,36/68;57.35%,39/68)高于常规脑电图(29.41%,20/68;38.24%,26/68),P<0.05。结论视频脑电图监测应用在继发性癫痫诊断中,可提高检出阳性率,能明确不同部位、不同类型发病情况。

Objective To investigate the diagnostic value of video electroencephalography monitoring for secondary epilepsy.Methods A total of 68 patients with secondary epilepsy admitted to Zhumadian Central Hospital from January 2019 to June 2022 were selected,and the patients underwent video electroencephalography and routine electroencephalography.The detection results of secondary epilepsy,epileptiform discharge,abnormal electroencephalography signals,synchronous seizures,and epileptiform waves in different areas were compared between the two monitoring methods.Results Among the 68 patients,the positive rate of secondary epilepsy detected by video electroencephalography(94.12%,64/68)was higher than that detected by routine electroencephalography(82.35%,6/68),P<0.05.The rates of epileptifrom discharge,abnormal electroencephalography signals and synchronous seizures detected by video electroencephalography(48.53%,33/68;89.71%,61/68;35.29%,24/68)were higher than those detected by routine electroencephalography(35.29%,24/68;76.47%,52/68;13.24%,9/68),P<0.05.The detection rate of epileptiform waves in the deep white matter area and cortical area of the brain lobe detected by video electroencephalography(52.94%,36/68;57.35%,39/68)were higher than those detected by routine electroencephalography(29.41%,20/68;38.24%,26/68),P<0.05.Conclusions The application of video electroencephalography monitoring in the diagnosis of secondary epilepsy can increase the positive rate of detection,and can clarify areas and types of seizure.
