1例抗磷脂综合征伴多发动脉血栓患者的抗栓治疗分析与药学监护

Analysis of antithrombotic therapy and pharmaceutical care for a patient with antiphospholipid syndrome and multiple arterial thrombosis

目的 分析探讨临床药师参与的1例抗磷脂综合征伴多发动脉血栓患者的抗栓治疗方案及药学监护,为临床合理使用抗栓药物提供参考。方法 临床药师参与1例因抗磷脂综合征导致腹主动脉、双肾动脉等多发动脉血栓患者的围术期诊疗,根据患者病理生理情况、辅助检查、抗栓药物特点等给出抗栓药物治疗方案建议。结果 医师采纳了临床药师提出的药物治疗及药学监护建议,患者预后良好,随访未出现血栓复发及出血等不良事件。结论 临床药师可以从药学专业知识的角度协助医师为患者制定个体化治疗方案,保障临床用药的合理性。

Objective In order to provide reference for rational use of antithrombotic drugs in clinical practice,the antithrombotic treatment plan and pharmaceutical care by clinical pharmacist in a patient with antiphospholipid syndrome and multiple arterial thrombosis were analyzed and discussed.Methods Clinical pharmacist participated in the treatment of a patient with multiple arterial thrombosis such as abdominal aorta and bilateral renal artery caused by antiphospholipid syndrome.According to the pathophysiology of patients,auxiliary examination and antithrombotic drug characteristics,the suggestions of antithrombotic treatment were proposed.Results The physicians adopted the suggestions of drug treatment and pharmaceutical care proposed by clinical pharmacists,and the patient had a good prognosis.No adverse events such as thrombosis recurrence and bleeding occurred during follow-up.Conclusion Clinical pharmacists can assist physicians in formulating individualized treatment plans for patients from the perspective of pharmaceutical professional knowledge to ensure the rationality of clinical drug use.
